武汉市某医院“医院——社区——家庭”延伸护理模式的实践与思考

摘　　要：随着护理服务领域范围的不断扩大,出院患者对进一步治疗与康复的需求不断增强,延伸护理模式成为综合性医院的一个关注重点及工作方向。武汉市某医院通过构建医院——社区——家庭延伸护理服务中心,开展社区服务支持,为出院患者提供居家护理服务,建立出院患者随访机制,成立专病俱乐部,积极开展志愿护理服务等,取得了显著成效。延伸护理服务模式有利于促进患者康复进程,提高患者满意度及对健康知识的知晓率。在提高医院社会效益的同时,还促进社区医疗的发展和社区护理水平不断提升。With the expansion of the nursing service fields,discharged patients＇ requirements about further treatments and rehabilitations are increasing. Therefore,hospital- community- family extended nursing model has become a focus and working direction of the comprehensive hospital. Thus,we did many explorations and practices including： construct hospital- community- family extended nursing service center,establish the community service support mode,provide home care services,establish a mechanism for discharged patients follow- up,form patient-club,carry out voluntary care services,so that we have achieved demonstrable results. Hospital- community- family extended nursing model can promote patients recovery processes,improve patient satisfaction and awareness of health knowledge. It can raise the social benefits of hospitals and can promote the development of community health care,promote the level of community care.
