慢性乙型肝炎肝纤维化指标与HBV-DNA的相关性

摘　　要：目的 探讨慢性乙型肝炎血清肝纤维化指标与HBV-DNA的相关性。方法 收集我院2010年3月~2013年5月收治的慢性乙型肝炎患者480例,将其按HBV-DNA水平分为5组（A组HBV-CAN〈103IU/L,B组103IU/L≤HBV-CAN〈105IU/L,C组105IU/L≤HBV-CAN〈106IU/L,D组106IU/L≤HBV-CAN〈108IU/L,E组HBV-CAN≥108IU/L）,比较其血清HA、PCⅢ、Ⅳ-C、LN水平和肝纤维化程度,并进行HBV-DNA与肝纤维化指标的Spearman相关性分析。结果 不同HBV-DNA水平患者肝纤维化指标HA、PCⅢ、Ⅳ-C、LN差异均具有统计学意义（P〈0.05）;HBV-DNA与肝纤维化指标相关性分析发现,HBV-DNA与HA、PCⅢ、Ⅳ-C、LN相关系数分别为0.874、0.815、0.748和0.789（均P〈0.05）。结论 慢性乙型肝炎病毒载量HBV-DNA与肝纤维化指标HA、PCⅢ、Ⅳ-C、LN之间存在相关关系,但乙肝肝纤维化判断应联合转氨酶等综合分析。Objective To explore the correlation between serum liver fibrosis indexes and HBV-DNA of patients with hepatitis B virus （HBV）. Methods 480 patients with chronic HBV to receive treatment in our hospital from March 2010 to May 2013 were collected and divided into five groups according to their HBV-DNA levels （group A： HBV-CAN 〈 103 IU/L; group B： 10^3 IU/L ≤ HBV-CAN 〈 10^5 IU/L; group C： 10^5 IU/L ≤ HBV-CAN 〈 10^6 IU/L; group D： 10^6 IU/L ≤ HBV-CAN 〈 10^8 IU/L; group E： HBV-CAN ≥ 10^8 IU/L）. Their serum HA, PCⅢ, Ⅳ-C, and LN levels as well as severity of liver fibrosis were compared and Spearman correlation analysis was made between HBV-DNA and liver fibrosis indexes. Results The differences in fibrosis indexes HA, PC Ⅲ, Ⅳ-C, and LN of patients at different HBV-DNA levels were statistically significant （P 〈 0.05）. The analysis on correlation between HBV-DNA and liver fibrosis indexes showed that the correlation coefficients of HBV-DNA with HA, PCⅢ, Ⅳ-C, and LN was 0.874, 0.815, 0.748,and 0.789, respectively （all P 〈 0.05）. Conclusion There is a correlation between the HBV-DNA load in patients with chronic HBV and the fibrosis indexes HA, PC Ⅲ, Ⅳ -C and LN, but judgment of HBV fibrosis should be based on comprehensive analysis of transaminase and other factors.
