自固定网片与聚丙烯平片用于成人TAPP的效果比较研究

摘　　要：目的 比较自固定网片和聚丙烯平片在经腹腹腔镜疝修补术（TAPP）中的安全性和有效性,探讨自固定网片的临床应用价值。方法 将2013年1~7月我院收治的83例成人单侧腹股沟疝行TAPP术患者随机分为自固定网片组（A组,43例）和聚丙烯平片组（B组,40例）。比较两组手术时间、术后住院时间、术后第3 d疼痛分数（VAS）、2 w内恢复正常非限制性活动人数、住院期间的医疗费用、异物不适感、慢性疼痛发生率和复发率,两组随访6~12个月。结果 在手术时间、术后3 d VAS、术后2 w恢复正常非限制性活动人数、异物感、慢性疼痛发生率上比较,A组均优于B组（P〈0.05）,而住院费用两组相当。结论 在TAPP中使用柯惠自固定网片治疗成人腹股沟疝安全有效,患者获益较大,值得推广。Objective To compare the safety and efficacy of self-gripping mesh and polypropylene flat in traMs-abdominal pro-peritoneal repair （TAPP） and explore the clinical curative effect of self-gripping mesh. Methods 83 adult patients with unilateral inguinal hernia to receive TAPP operation were randomly divided into self-gripping mesh group （group A, n = 43） and polypropylene flat sheet group （group B, n = 40）. The operation time, postoperative hospitalization time, postoperative third-day pain scores （VAS）, numbers of patients recovering to normal and non-limited activities within two weeks, medical expenses during the hospitalization, body discomfort due to foreign article, and chronic pain incidence and recurrence rate were compared between the two groups. The patients in both groups were followed up for 6-12 months. Results In terms of the operation time, postoperative three-day VAS scores, the numbers of patients recovering to normal and non-limited activities within two weeks, body discomfort due to foreign article, and chronic pain incidence, group A was better than group B （P 〈 0.05）, and the hospitalization expenses in two groups were equivalent. Conclusion Parietex ProGrip self-gripping mesh applied to adult patients with inguinal hernia is safe and effective, and it is beneficial to the patients, so it is worthy of promotion.
