摘　　要：目的 评价超选择性栓塞治疗有假性动脉瘤的高流量性阴茎异常勃起的技术与疗效。 方法 2006年6月—2012年9月我院介入科共收治有假性动脉瘤的高流量性阴茎异常勃起患者3例, 采用明胶海绵进行超选择插管栓塞, 栓塞术后评价患者的阴茎勃起状态, 并分别于术后3、6个月对患者进行彩色多普勒超声随访, 对勃起功能随访20-92个月, 根据国际勃起功能指数5（IIEF5）评分标准进行评价。结果 超选择动脉插管及栓塞均获得成功。12个月内随访发现1例患者因过度栓塞出现轻度勃起功能障碍, IIEF5评分15分, 2例患者勃起功能正常, 评分22分和23分, 随访期内所有患者未再复发。结论 超选择栓塞对治疗有假性动脉瘤的高流量性阴茎异常勃起安全、有效, 正确超选择靶血管及合适的栓塞剂用量是成功的关键。Objective To evaluate the effectiveness and techniques of superselective artery embolization for treatment of high-flow priapism with pseudoaneurysms. Methods From June 2006 to September 2012, 3 patients with high-flow priapism caused by pseudoaneurysm were enrolled. The pseudoaneurysms were treated by superselective embolization with gelatin sponges. The priapism was evaluated after the procedure, and the patients were followed up by ultrasound 3 and 6 months after the embolization. The erectile functions were assessed by the International Index of Erectile Function 5 （IIEF-5） for 20—92 months. Results Superselective embolization was successfully accomplished in all 3 cases. In followed-up 12 months, 1 patient who had required excessive embolization had mild erectile dysfunction on follow-up, with IIEF-5 score of 15, 2 patients had normal erectile function, scoring 22 and 23 points. All patients did not recur. Conclusion Superselective embolization is a safe and effective procedure in treatment of high-flow priapism with pseudoaneurysm. Proper superselection of the target artery and suitable use of the embolization agents are the key to success.
