生殖股神经在输尿管手术中的应用解剖

摘　　要：目的为临床腹膜后隙输尿管诊疗操作时生殖股神经的定位和避免伤及生殖股神经提供应用解剖学资料。方法对19具(38侧)成人湿性防腐尸体进行生殖股神经的解剖学观察、测量。结果在腰大肌穿出点平面,穿出点位于输尿管内侧占81.67%(31/38)、位于输尿管外侧占18.33%(7/38);在骼嵴最高点平面,生殖股神经位于输尿管外侧占69.87%(26/38),位于输尿管内侧占31.13%(12/38);在骼总血管分叉平面,生殖股神经全部行于输尿管的外侧。结论为避免损伤生殖股神经,在输尿管起始段手术应从后外侧分离较安全;在输尿管第二狭窄附近手术应从后内侧分离较为安全。Objective To provide the anatomical data for avoiding the genitofemoral nerves injury in ureteral operation.Methods The genitofemoral nerve of 38 sides of 19 adult corpses fixed in formalin were observed.Results On the exit points of genitofemoral nerves in psoas major,the percentage of exit points of genitofemoraln nerves in the interior or exterior of urters were approximate 81.67 %（31/38）and 18.33%（7/38）.On the plane of the iliac crest culmination,the percentage of genitofemoral nerves in the interior of ureters was 31.13%（12/38）,that in the exterior was 69.87%（26/38）.On the furcation level plane of the common iliac vessels,all the genitofemoral nerves were on the posterior of ureters.Conclusion The operation on the origination of the ureters is safer by performing at the posterior and exterior part.Inversely,the operation on the second narrow of ureters is safer when performed on the posterior and interior part.
