摘　　要：目的：探索低碳水化合物饮食对改善肥胖型多囊卵巢综合征（PCOS）患者不孕的效果并探讨其相关机制。方法：将符合纳入和排除标准的151例PCOS患者分为干预组和对照组。干预组给低碳水化合物饮食,干预时间为8周,分为减重期（0～4周）和维持期（〉4～8周）;对照组仅给予一般性的饮食建议,观察患者干预前后体重指数（BMI）的变化和妊娠情况。结果：干预组患者在减重期BMI由（27.7±1.7）kg/m2降至（26.3±0.4）kg/m2（P〈0.05）,在维持期降至25.7±1.5kg/m2（P〈0.05）;对照组在干预前后BMI由27.6±1.8kg/m2降低至27.4±1.9kg/m2;干预组和对照组BMI的变化差异有统计学意义（P〈0.05）。另外干预组妊娠率为18%,对照组无一例妊娠,差异有统计学意义（P〈0.05）。结论：低碳水化合物饮食能有效促进PCOS患者的体重下降,进而改善不孕症状。Objective：To investigate the effect of a low carbohydrate diet on infertility among overweight and obese women with polycystic ovary syndrome（PCOS）.Methods：A total of 151 women with a clinical diagnosis of PCOS were divided into nutrition intervention group and control group.In the intervention group,patients received a low carbohydrate diet for 8weeks,and the intervention included weight loss and maintenance phase.In the control group,patients received the general dietary recommendations.Body mass index（BMI）and pregnancy outcome were observed before and after a 8-weeks period in these two groups.Results：BMI decreased from（27.7±1.7）kg/m2 to（26.3±0.4）kg/m2 during the weight loss period,and to（25.7±1.5）kg/m2 during the weight maintenance period in the intervention group.The mean level of BMI was（27.6±1.8）kg/m2 and（27.4±1.9）kg/m2 from baseline to 8weeks in the control group.There was a significant reduction in BMI between the intervention group and the control group（P〈0.05）.Moreover,the pregnancy rate of intervention group was 18%,with a significant difference compared with the control group（P〈0.05）.Conclusion：The low carbohydrate diet could effectively promote weight loss in PCOS patients,and improve the symptom of infertility induced by PCOS.
