摘 要:目的:探讨不同糖耐量状态下绝经后女性血清氧化低密度脂蛋白(ox-LDL)水平及其相关影响因素。方法:选取120例绝经后女性,其中糖耐量正常(NGT组)40例、糖耐量受损(IGR组)40例及2型糖尿病(T2DM组)40例。采用ELISA检测血清中ox-LDL水平,分析比较各组ox-LDL的水平及其与血脂、血糖(FBG)、收缩压(SBP),舒张压(DBP)、同型半胱氨酸(Hcy)的相关性。结果:与NGT组比,T2DM组及IGR组均具有较高的ox-LDL、LDL-C、SBP、DBP水平(P<0.05),且T2DM组血Hcy明显高于NGT组(P<0.01)。与IGR组相比,T2DM组具有较高的ox-LDL、TG、TCH、LDL-C、FBG及低HDL-C水平(P<0.05),两组间的SBP、DBP、Hcy差异无统计学意义(P>0.05)。多元线性回归分析ox-LDL与Hcy、LDL-C、FBG、SBP呈正相关,与HDL-C呈负相关(P<0.05),各相关指标对ox-LDL水平影响的标准化回归系数(β)分别为0.394(FBG)、0.213(Hcy)、0.177(SBP)、-0.139(HDL-C)和0.125(LDL-C)(P<0.05)。结论:随着糖耐量异常程度的加重,血清ox-LDL水平也呈升高趋势,而且高血糖、高Hcy、高血压及血脂紊乱是ox-LDL的独立危险因素。Objective: To explore the levels of serum oxidized low- density lipoprotein( ox- LDL) among postmenopausal women with different glucose tolerance and the related influencing factors. Methods: A total of 120 postmenopausal women were selected and divided into normal glucose tolerance( NGT) group( 40 women),impaired glucose tolerance( IGT) group( 40 women),and type 2 diabetes mellitus( T2DM) group( 40 women). ELISA was used to detect the levels of serum ox- LDL; the correlations between serum ox- LDL levels and blood lipid,fasting blood glucose( FBG),systolic blood pressure( SBP),diastolic blood pressure( DBP),homocysteine( Hcy)in the three groups were analyzed and compared. Results: Compared with NGT group,the levels of ox- LDL,LDL- C,SBP,and DBP were statistically significantly higher than those in T2 DM group and IGR group( P 〈0. 05),the level of Hcy in T2 DM group was statistically significantly higher than that in NGT group( P 〈0. 01). Compared with IGT group,the levels of ox- LDL,TG,TCH,LDL- C,and FBG were higher and the level of HDL- C was lower in T2 DM group( P 〈0. 05),there was no statistically significant difference in SBP,DBP,and Hcy between IGT group and T2 DM group( P 〉0. 05). Multivariate linear regression analysis showed that ox- LDL was positively correlated with Hcy,LDL- C,FBG,SBP; ox- LDL was negatively correlated with HDL- C( P 〈0. 05),the standardized regression coefficients( β) of the effects of related indexes on ox- LDL level were 0. 394( FBG),0. 213( Hcy),0. 177( SBP),- 0. 139( HDL- C),and 0. 125( LDL- C),respectively( P 〈0. 05). Conclusion: With the aggravation of abnormal glucose tolerance,serum ox- LDL level shows an increasing trend,hyperglycaemia,hyperhomocysteinemia,hypertension,and dyslipidemia are independent risk factors of ox-LDL.
