摘  要:目的:检测慢性阻塞性肺疾病(COPD)急性加重期(AECOPD)患者的血浆同型半胱氨酸(Hcy)、一氧化氮(NO)、白细胞介素-8(IL-8)、超敏C反应蛋白(hs-CRP)水平,探讨其对COPD的作用机制。方法:AECOPD患者200例,年龄及其性别与其匹配的健康对照组180例。测定200例AECOPD患者血浆Hcy、NO、IL-8、hs-CRP水平,并与健康对照组进行比较。结果:1NC组、AECOPD组一般临床资料的比较显示两组患者的性别、年龄、血压比较差异无统计学意义(P>0.05)。2NC、AECOPD两组临床资料比较显示AECOPD组患者血浆总同型半胱氨酸(t Hcy)、IL-8及hs-CRP水平明显高于NC组(P<0.01)。NO与NC组比较显著性降低(P<0.01)。3t Hcy与IL-8及hs-CRP呈正相关,与NO呈负相关。结论:COPD患者血浆存在高Hcy和低NO水平,同时伴有IL-8升高,三者参与了COPD的发生与发展,是衡量COPD病情严重程度和判断患者的疗效及预后的重要指标。Hcy通过多种途径参与COPD的发生与发展,减少Hcy的药物可能能延缓COPD的进展。Objective: Detection of chronic obstructive pulmonary disease (COPD) (AECOPD) in patients with acute exacerbation of plasma homocysteine (Hcy), nitric oxide (NO), IL-8, high sensitive C reactive protein (hs-CRP) level, and to explore its action mechanism on COPD. Methods: 200 cases of AECOPD patients, age and gender matched healthy control group of 180 cases. Determination of plasma in 200 patients with AECOPD Hcy, NO, IL-8, HS and CRP levels, and compared with control group.Results:Group NC, group AECOPD with clinical data comparison shows that two groups of patients with gender, age, blood pressure difference was not significant (P〉0.05). The NC, AECOPD two groups of clinical data comparison indicated that the AECOPD group patients plasma tHcy, IL-8 and hs-CRP levels were significantly higher than that of group NC (P〈0.01). NO and NC group was significantly lower (P〈0.01). The tHcy and IL-8 and hs-CRP were positively correlated, negatively related with NO.Conclusion:Plasma COPD in patients with high Hcy and low NO levels, accompanied by the increase of IL-8, the three participated in the occurrence and development of COPD, is an important index to evaluate the severity of COPD and judge curative effect and prognosis of the patients. The occurrence and development of Hcy involved in COPD through a variety of ways, reduce Hcy drugs may delay the progress of COPD.
