摘  要:目的探讨甲泼尼龙联合高压氧治疗对大鼠脊髓损伤的保护作用,以及对自噬基因(Beclin-1)和TGF-β蛋白表达的调节作用。方法将实验动物随机分入假手术组、模型组、甲泼尼龙组和甲泼尼龙联合高压氧治疗组(联合治疗组),每组15只。采用Allen法建立大鼠脊髓损伤模型。模型组大鼠予腹腔内注射0.9%氯化钠溶液,甲泼尼龙组大鼠予腹腔内注射甲泼尼龙30 mg/kg治疗,联合治疗组大鼠予腹腔内注射甲泼尼龙30mg/kg和0.2 MPa氧压力的高压氧治疗;3组大鼠均持续处理7d后结束实验,解剖大鼠取材,进行病理切片分析。检测大鼠血清中IL-1和IL-6水平,采用Western印迹法测定大鼠脊髓组织Beclin-1和TGF-β蛋白表达量。结果组织病理学检查结果显示,假手术组大鼠脊髓结构完整,神经细胞生长正常;模型组大鼠脊髓组织灰白质界限不清楚,神经细胞凋亡、坏死;甲泼尼龙组和联合治疗组大鼠脊髓组织灰白质界限清晰,结构完整,神经细胞发生明显的空泡样变性,甲泼尼龙组大鼠脊髓组织病理损伤程度大于联合治疗组。模型组、甲泼尼龙组和联合治疗组大鼠血清IL-1和IL-6水平,以及大鼠脊髓组织中Beclin-1和TGF-β蛋白相对表达量均显著高于假手术组(P值均<0.05);甲泼尼龙组和联合治疗组大鼠血清IL-1和IL-6水平,以及大鼠脊髓组织中Beclin-1和TGF-β蛋白相对表达量均显著低于模型组(P值均<0.05);联合治疗组大鼠血清IL-1和IL-6水平,以及大鼠脊髓组织中Beclin-1和TGF-β蛋白相对表达量均显著低于甲泼尼龙组(P值均<0.05)。结论甲泼尼龙联合高压氧治疗对大鼠脊髓损伤具有一定的保护作用,其作用机制可能是通过抑制脊髓损伤后Beclin-1和TGF-β蛋白表达来减轻脊髓损伤,从而抑制脊髓损伤瘢痕产生。Objective To study the effect of methylprednisolone combined with hyperbaric oxygen on spinal cord injury in rats and expression of Beclin-1 and transforming growth factor (TGF)-β protein. Methods Experimental animals were randomly divided into four groups; sham operation group, model group, methylprednisolone group and methylprednisolone combined with hyperbaric oxygen group (combined treatment group). There were 15 rats in each group. Spinal cord injury was induced by Allen's method. Intraperitoneal injection of normal saline was given in the model grouP. Methylprednisolone (30 mg/kg) was intraperitoneally injected in the methylprednisolone group. Methylprednisolone (30 mg/kg) and 0.2 MPa hyperbaric oxygen were given in the combined treatment group. All the treatments continued for 7 d. Then myeloid tissue was taken from the rats and pathological section was made. Serum interleukin (IL)-I and IL-6 were detected. Beclin-1 and TGF-β protein levels were measured by Western blot. Results Pathological results showed that the structure of spinal cord was intact and nerve cells grew normally in the sham operation group; the boundaries between spinal cord gray matter and white matter were unclear, and neuronal apoptosis and necrosis appeared in the model group; the boundaries between spinal cord gray matter and white matter were clear and tissue structural integrity presented, but vacuolar degeneration was obvious in nerve cells in the combined treatment group~ spinal cord injury in the methylprednisolone group was more severe than that in the combined treatment group. Compared with those in the sham operation group, serum IL-1 and IL-6 levels and protein expression of Beclin-1 and TGF-β in the model group, methylprednisolone group and combined treatment group were significantly increased (all P〈0.05).
