针灸董氏奇穴“下三皇”治疗慢性肾炎蛋白尿临床对照研究

摘　　要：目的：比较针灸董氏奇穴＂下三皇＂与西药盐酸苯那普利治疗慢性肾炎蛋白尿的临床疗效差异,为治疗慢性肾炎蛋白尿提供有效疗法。方法：将106例肾炎蛋白尿患者随机分为针灸组和西药组,每组53例。针灸组给予针灸＂下三皇＂（董氏奇穴将天皇副穴＂肾关＂＂地皇＂＂人皇＂称为＂下三皇＂）治疗,每日1次,每治疗10次休息2天;西药组予口服盐酸苯那普利治疗,每次5~10mg,每日1次。两组均2个月为一疗程,治疗1个疗程后分别观察两组患者的临床疗效,同时观察两组患者治疗前后的尿沉渣红细胞计数、24h尿蛋白定量和内生肌酐清除率。结果：针灸组总有效率为84.9%（45/53）,优于西药组的52.8%（28/53,P〈0.01）。治疗后两组患者尿沉渣红细胞计数、24h尿蛋白定量水平均较治疗前下降（P〈0.01,P〈0.05）,且针灸组低于西药组（P〈0.05,P〈0.01）;治疗后针灸组内生肌酐清除率高于治疗前（P〈0.05）,且高于西药组（P〈0.05）,而西药组治疗前后差异无统计学意义（P〉0.05）。结论：针灸董氏奇穴下三皇治疗慢性肾炎能减少蛋白尿和改善肾功能,疗效优于西药盐酸苯那普利。Objective To compare the difference on the clinical efficacy in treatment of chronic nephritis albuminuria between acupuncture and moxibustion at＂Xiasanhuang＂,DONG＇s extra points and benazepril hydrochloride so as to provide the effective therapeutic method for the treatment of chronic nephritis albuminuria.MethodsOne hundred and six cases of nephritis albuminuria were randomized into an acupuncture and moxibustion group and a western medication group,53 cases in each one.In the acupuncture and moxibustion group,acupuncture and moxibustion were applied to＂Xiasanhuang＂（Tianhuang fuxue namely ＂shenguan＂,＂Dihuang＂,＂Renhuang＂）,once a day,the interval of 2days once every 10 treatments.In the western medication group,benazepril hydrochloride was prescribed for oral administration,5to 10 mg each time,once a day.The treatment of 2months made one session in the two groups.After 1session treatment,the clinical efficacy,red blood cell count（RBC）of urinary sediment,24 hurine protein quantitation and creatinine clearance rate（Ccr）before and after treatment were observed in the two groups.Results The total effective rate was 84.9%（45/53）in the acupuncture and moxibustion group,superior to 52.8%（28/53）in the western medication group（P〈0.01）.After treatment,RBC of urinary sediment and 24 hurine protein quantitation were reduced as compared with those before treatment（P〈0.01,P〈0.05）,and the results in the acupuncture and moxibustion group were lower than those in the western medication group（P〈0.05,P〈0.01）.After treatment,Ccr in the acupuncture and moxibustion group was higher than that before treatment（P〈0.05）and higher than that in the western medication group（P〈0.05）.Ccr was not different significantly before and after treatment in the western medication group（P〉0.05）.Conclusion Acupuncture and moxibustion at DONG＇s extra points（Xiasanhuang）reduce proteinuria and improve kidney function in patients of chronic nephritis and the efficacy is b
