偏头痛针灸治疗成本效果分析问卷调查研究

摘　　要：目的：通过问卷调查的方式了解医患双方对偏头痛针灸治疗成本效果的评价。方法：采用问卷调查方式,分别编制患者问卷及医生问卷,利用网络平台,分别公开发布两份调查问卷,在线填写,后台实时回收,进行数据统计。结果：回收有效患者问卷696份,医生问卷114份。结果显示：1常规针刺、常规针刺＋电针、穴位埋线3种针灸疗法预防性治疗偏头痛的直接成本（包括治疗及材料费）分别为109元/次、152元/次、238元/次,往返医院的交通费市内为42元/次、外埠患者城市间为193元/次;间接成本包括误工费（208元/次）、治疗的时间成本（穴位埋线治疗耗时10~30min/次,每1~3周治疗1次;其他两种治疗方法耗时20~50min/次,每周治疗1~5次）、往返医院路途时间成本（市内多为1~3h/次、城市间4h/次）。从整个疗程来看,穴位埋线疗法降低了治疗频率,总成本相对较低。2对医患双方选择不同针灸疗法治疗偏头痛的原因进行调查,患者问卷结果显示：选择穴位埋线疗法的患者与选择其他两种针灸方法的患者关注点明显不同,疗效好且持久,同时又节约成本,是穴位埋线疗法的主要优势。医生问卷结果显示：选择穴位埋线的主要原因是＂单次治疗疗效持久＂（87.5%）和＂减少患者就诊次数,节约成本＂（75.0%）。3未被广为了解,是阻碍穴位埋线疗法推广使用的主要原因。结论：穴位埋线疗法是一种理想的偏头痛预防性治疗措施,成本效果相对较高,但目前未能得到广泛的应用。今后应加强宣传、培训力度,有针对性地开展穴位埋线疗法治疗偏头痛的推广工作。Objective By using questionnaire method to understand the evaluation regarding cost-effectiveness of acupuncture for migraine in both doctors and patients.Methods By using questionnaire method,questionnaires for doctors and patients were made respectively;by using network platform,two sets of questionnaires were publicly released,which were filled in online.The results were real-time background collected and then analyzed.Results Totally 696 effective questionnaires for patients and 114 questionnaires for doctors were collected.The results indicated that1 the direct cost（including treatment fee and material fee）for preventive treatment of migraine was 109 yuan per time in regular acupuncture,152 yuan per time in regular acupuncture ＋ electroacupuncture（EA）and 238 yuan per time in acupoint catgut embedding.The travelling expense was 42 yuan per time within the city and 193 yuan per time between cities.The indirect cost included cost for loss of working time（208 yuan per time）,cost for treatment time（acupoint catgut embedding：10to 30 min per time,once one to three weeks;the remaining two treatments：20to 50 min per time,once to 5times every week）,time cost for arriving and departing hospital（1to 3hours per time within the city and 4hours per time between cities）.From the angle of treatment course,acupoint catgut embedding reduced the treatment frequency,leading to a lower total cost.2Among the reasons to select different acupuncture methods to treat migraine,the results of patient questionnaire indicated that the focus in the patients who selected acupoint catgut embedding was different from that in the patients who selected two other treatments.The main advantage of acupoint catgut embedding was superior and lasting efficacy with low cost.The results of doctor questionnaire indicated the main reason to select acupoint catgut embedding was＂lasting efficacy after single treatment＂（87.5%）and＂lower frequency of treatment＂（75.0%）.3The main reason to obstruct the popularization o
