摘 要:目的分析镍钛摇椅弓配合Ⅱ类牵引矫治成人安氏Ⅱ类1分类错的临床疗效。方法采用镍钛摇椅弓配合Ⅱ类牵引对14例成人安氏Ⅱ类1分类患者进行矫治,并对矫治前后的头影测量结果进行统计学分析。结果 14例患者治疗后双侧磨牙呈中性关系,尖窝锁结关系良好,覆牙合、覆盖正常,软组织侧貌改善。X线头影测量显示,治疗后U1-SN角减小13.66°,UI-NA和L1-NB距离分别减小5.51 mm和2.16 mm,前牙覆、覆盖分别减小1.69 mm和4.20 mm,除下颌中切牙明显压低外(t=-6.04,P<0.001),下颌磨牙还出现了一定量的伸长(t=10.26,P<0.001),但面高增加无统计学意义(t=1.44,P=0.18)。结论镍钛摇椅弓配合Ⅱ类牵引是矫治成人安氏Ⅱ类1分类错的有效方法,对其可能产生的副作用加以控制,可获得满意的治疗效果。Objective The objective of this study was to investigate the effect of Angle Ⅱ1 malocclusion correction in aduhs with the use of NiTi reversed curve arch wire and Class Ⅱ elastics. Methods 14 adult patients with Angle Ⅱ1 malocclusion were treated with the use of NiTi reversed curve arch wire and Class Ⅱ elastics purely. The results of eephalometrie measurements before and after treatment were statistically analyzed. Results 14 patients had normal overbite and overjet, good intereuspid relationship and better facial profile after treatment. The results of cephalometric measurements showed that U1-SN decreased by 13.66°, UI-NA decreased by 5.51 mm, L1-NB decreased by 2.16 mm, anterior overbite decreased by 1.69 mm, and overjet decreased by 4.20 mm. The lower incisors were intruded ( t = - 6.04, P 〈 0.001 ) , while the lower first molars were extruded ( t = 10. 26, P 〈 0.001 ). The face height had no obvious changes compared with that of pre-treatment ( t = 1.44, P = 0.18 ). Conclusion NiTi reversed curve arch wire with Class Ⅱ elastics is an effective way to treat Angle Ⅱ1 malocclusion in adult patients. Dentists need to control the side effects to acquire a satisfactory results.
