鼻尖部及其邻近部位创伤缺损的急诊处理与美容修复

摘　　要：目的探讨鼻尖部及其邻近部位创伤缺损的急诊处理与美容修复方法。方法自2010年1月至2014年8月，对收治的9例创伤后鼻尖部及其邻近部位缺损患者，在清创后采用上臂内侧带蒂皮瓣Ⅰ期修复创面，3周后断蒂，实施缺损的鼻部各美学亚单位的修复重建。结果9例上臂内侧皮瓣均100％成活，伤口Ⅰ期愈合。皮瓣断蒂行Ⅰ期修复重建后，4例外形良好；5例外形稍显臃肿，对称性欠佳；术后3个月，经局部修整，外形恢复良好。随访4～12个月，外形良好，患者满意。结论上臂内侧皮瓣是急诊修复鼻尖部及其邻近部位创伤缺损的一种可供选择的方法，可为修复重建鼻尖部位缺损的各个美学亚单位提供充足的组织量。Objective To study the method of emergency treatment and plastic repair of traumatic defects on nasal tip and its periphery. Methods From January 2010 to August 2014, 9 patients with traumatic defects on nasal tip and its periphery were treated. After debridement, pedicle flaps from medial upper arm were employed to repair the defects on the first stage. The damaged aesthetic subunits of nose were constructed at 3 weeks after pediele cutting. Results All flaps on 9 patients survived with primary wound healing. After reconstruction of aesthetic subunits of nose, 4 cases were satisfied with appearance; 5 cases were unsatisfied with slight swelling appearance and asymmetry which recovered by local plastic operations at 3 months postoper- atively. All patients were followed up from 4 to 12 months and all were satisfied with the results. Conclusion The medial upper arm flap might be a selected method to emergency repair the traumatic defects on nasal tip and its periphery, and could supply enough tissue quantity to repair and reconstruct each plastic subunits on nasal tip.
