绝经后女性2型糖尿病患者合并NAFLD的血脂特点分析

摘　　要：目的:研究绝经后女性2型糖尿病合并非酒精性脂肪肝病(NAFLD)的血脂特点。方法:住院绝经后2型糖尿病女性患者117例,其中单纯2型糖尿病患者(T2DM)61例,2型糖尿病合并NAFLD患者(T2DM+NAFLD)56例。分别检测两组患者的空腹血糖(FBG)、血脂、血压、体重指数(BMI)、糖化血红蛋白(Hb A1C)和肝功能。结果:绝经后女性T2DM组与T2DM+NAFLD组年龄、总胆固醇、低密度脂蛋白胆固醇、收缩压、舒张压、FBP、Hb A1C比较差异无统计学意义(P>0.05)。T2DM+NAFLD组甘油三酯(TG)、BMI、谷草转氨酶、谷丙转氨酶、γ-谷氨酰转肽酶明显高于T2DM组,高密度脂蛋白胆固醇明显低于T2DM组(P<0.01)。Logistic回归分析显示,BMI增加和TG升高是绝经后女性2型糖尿病合并NAFLD的危险因素。结论:TG升高和体重增加与绝经后2型糖尿病女性合并NAFLD的发生密切相关。Objective： To study the lipid characteristics of postmenopausal women with type 2 diabetes mellitus（T2DM） combined nonalcoholic fatty liver disease（NAFLD）.Methods： This study included 117 cases hospitalized postmenopausal women with T2 DM, which were 61 T2 DM patients and 56 T2 DM combined NAFLD patients. The fast blood glucose（FBG）, blood lipid, blood pressure, body mass index（BMI）, glycosylated hemoglobin（Hb A1C） and liver function of all the patients were respectively detected.Results：There had no statistical signifi cance about age, total cholesterol, low density lipoprotein cholesterin, systolic blood pressure, diastolic blood pressure, FBG, HbA 1C between T2 DM group and T2 DM combined NAFLD group（P〉0.05）. The triglyceride（TG）, BMI, glutamic oxalacetic transaminase, glutamic-pyruvic transaminase, γ-glutamyltranspeptidase of postmenopausal women in T2 DM combined NAFLD group were obviously higher than that of postmenopausal women in T2 DM group, but high density lipoprotein cholesterin was obviously lower（P〈0.01）. Logistic regression analysis showed that increasing of BMI and elevated TG are independent risk factors for postmenopausal women with T2 DM combined NAFLD. Conclusions： Elevated TG and weight increasing are closely related to postmenopausal women with T2 DM combined NAFLD.
