可脱卸弹簧圈早期栓塞术治疗颅内破裂动脉瘤的疗效观察

摘　　要：目的探讨可脱卸弹簧圈早期栓塞术治疗颅内破裂动脉瘤的临床疗效。方法 2011年1月至2014年1月间收治的120例(动脉瘤122个)颅内破裂动脉瘤患者,采用随机数字表法分为观察组和对照组,每组60例(动脉瘤61个)。观察组患者采用可脱卸弹簧圈早期栓塞治疗,对照组患者采用手术夹闭治疗,平均随访6个月,比较两组患者治疗后破裂动脉瘤的闭塞率及临床疗效。结果与对照组患者相比,观察组患者治疗后破裂动脉瘤的100%闭塞率明显提高(58.3%和40.0%,P=0.04),恢复良好率显著提高(93.3%和78.3%,P=0.02),表明可脱卸弹簧圈早期栓塞术治疗颅内破裂动脉瘤能够提高闭塞率,改善患者的预后质量。结论对于颅内破裂动脉瘤患者,早期应用可脱卸弹簧圈栓塞术治疗能够有效提高破裂动脉瘤的闭塞率,减少再次破裂出血的发生率,改善患者的预后。Objective The aim of this study is to explore the clinical efficacy of early endovascular embolization of ruptured intracranial aneurysms with detachable coils. Methods From January 2011 to January 2014,120 patients（ 122 cases） with ruptured intracranial aneurysms in People＇s Hospital of Hubei were enrolled in the study and randomly divived into the control group（ operation treatment by clipping） and the observation group（ early endovascular embolization with detachable coils）,60 patients（ 61 cases） per group. Patients in the control group accepted operation treatment by clipping; Patients in the observation group accepted early endovascular embolization with detachable coils. After follow-up for 6 mo,the occlusion rate of aneurysm rupture and clinical efficacy of the two groups were observed and compared. Results Compared with the control group,the 100% occlusion rate of aneurysm rupture was significantly increased（ 58. 3% vs 40. 0%,P = 0. 04）,and the good recovery rate was obviously improved（ 93. 3% vs 78. 3%,P = 0. 02） in the observation group. This results showed that the treatment of early endovascular embolization with detachable coils for ruptured intracranial aneurysms could increase the occlusion rate and improve the patients＇ prognosis. Conclusions For patients with ruptured intracranial aneurysms,treatment of early endovascular embolization with detachable coils can effectively increase the occlusion rate of aneurysm rupture,reduce the incidence of re-bleeding,and improve the prognosis.
