急性上呼吸道感染降钙素原和C反应蛋白检测的临床意义

摘　　要：目的探讨测定血清中降钙素原(PCT)和C反应蛋白(CRP)在急性上呼吸道感染诊断中的临床意义。方法将急性上呼吸道感染患者83例分为A组(细菌感染组,n=36)和B组(病毒等病原体感染组,n=47),分别测定其血清PCT和CRP浓度;以同期健康查体者30例为对照组。结果 83例上呼吸道感染患者的血清PCT,CRP浓度较30例对照组显著升高(P<0.01);A组患者血清PCT,CRP浓度较B组显著升高(P<0.01);在细菌性感染中血清PCT和CRP浓度呈正相关。血清PCT,CRP及两者联合诊断细菌感染的敏感度分别为84.38%,85.42%,97.22%,特异度分别为80.0%,75.00%,97.6%,阴性预测值分别为46.12%,64.29%,86.11%.结论联合检测血清PCT,CRP浓度可明显提高上呼吸道细菌感染诊断的可靠性,有益于临床治疗。[ ABSTRACT] Objective To discuss clinical significance of detecting blood procalcitonin （ PCT） and C-reactive protein（ CRP） in diagnosis of acute upper respiratory infection .Methods Eighty-three patients with acute upper respira-tory infection were classified into group A including 36 patients with bacterial infection and group B including 47 patients with virus and other pathogens infection .The densities of PCT and CRP of group A and group B were measured respec-tively.Thirty persons were selected as control group who did health examination at the same time .Results Densities of PCT and CRP of 83 patients with acute upper respiratory infection were significantly higher than those in normal control （P〈0.01）.Compared with group B,densities of PCT and CRP of group A increased significantly （P〈0.01）.The level of PCT was positively correlated to the level of CRP in the bacterial infection .The sensitivity of PCT ,CRP and the com-bined determination of PCT and CRP in diagnosing bacterial infection was 84.38%,85.42%and 97.22%respectively. The specificity was 80.0%,75.00% and 97.6%,respectively.And the negative predictive values are 46.12%,64. 29%and 86.11%,respectively.Conclusion The combined determination of PCT and CRP can improve the reliability of diagnosing bacterial infection of the acute upper respiratory tract and is beneficial to clinical treatment .
