冬虫夏草提取物联合甲泼尼龙治疗特发性肺纤维化疗效研究

摘　　要：目的：探讨冬虫夏草提取物联合甲泼尼龙治疗特发性肺纤维化的治疗效果。方法选取2012年1月～2013年12月间我院收治的特发性肺纤维化患者60例，随机分为2组，对照组口服甲泼尼龙剂量为0．5mg／（kg· d），4周后给药剂量调整为0．25mg／（kg· d），连续治疗4个月。观察组口服甲泼尼龙剂量为0．5mg／（kg· d），口服冬虫夏草提取物-百令胶囊剂量为60mg／（kg· d），4周后甲泼尼龙减半量维持，疗程均为4个月。结果观察组临床疗效总有效率为86．67％，对照组临床疗效总有效率为70．00％，观察组临床疗效总有效率明显优于对照组（P＜0．05）。两组患者经治疗后的动脉血气分析及FVC％pred，DLCO／VA％pred等肺功能指标较治疗前均明显提高（P＜0．05）。与对照组相比，观察组患者在治疗后动脉血氧分压（PaO2）及FVC％pred，DLCO／VA％pred等指标的提高幅度更为明显（ P＜0．05）。观察组肺部CT的改善情况明显优于对照组（ P＜0．05）。结论冬虫夏草提取物联合甲泼尼龙治疗特发性肺纤维疗效理想，值得在临床上推广应用。[ ABSTRACT] Objective To investigate the treatment effect of Cordyceps sinensis extract combined with methylprednisolone in the treatment of idiopathic pulmonary fibrosis .Methods Sixty cases of idiopathic pulmonary fibrosis received by our hospital in Jan .2012 to Dec.2013 were selected and divided into two groups randomly （30 cases per group）.In control group,the oral dose of methylprednisolone was 0.5mg/（kg· d） and 4 weeks later,the dosage was adjusted to 0.25mg/（kg· d）,while in observation group,oral methylprednisolone dose was 0.5mg/（kg· d）,oral administration of Cordyceps Extract-bailing capsule dosage was 60mg/（kg· d）,after 4 weeks,the dosage of meth-ylprednisolone was adjusted to half the amount of maintenance ,the treatment course was all 4 months.Results The total clinical cure rate of the observation group was 86.67%,while the total effect rate of the control group was 70%,the total effect rate of the observation group was significantly higher than that of the control group （P〈0.05）.After treatment,the arterial blood gas analysis and FVC%pred,DLCO/VA%pred and other indexes of pulmonary function of patients were significantly improved （P〈0.05）.Compared with the control group ,PaO2 and FVC%pred,DLCO/VA%pred and other indicators of patients in the observation group after treatment improved more obviously （P〈0.05）. So as the lung improvement showed on CT （P〈0.05）.Conclusion The therapeutic results of cordyceps sinensis extract in combination with methylprednisolone on idiopathic pulmonary fibrosis is ideal and the method is worthy of clinical application .
