摘　　要：目的调查慢性阻塞性肺疾病急性加重期(Acute Exacerbation of Chronic Obstructive Pulmonary Disease AECOPD)住院患者睡眠质量,并探讨影响其睡眠质量的相关危险因素。方法回顾性分析2013年1月至2014年1月我科确诊的95例老年AECOPD住院患者的临床资料,采用匹兹堡睡眠质量指数(PSQI)量表进行调查评估睡眠质量,并对影响患者睡眠质量的相关因素进行分析。结果本组95例老年AECOPD住院患者中63例发现有睡眠障碍。年龄、经济负担、夜间治疗、对AECOPD相关知识了解程度、心理因素(对死亡的焦虑和抑郁)与老年AECOPD住院患者睡眠质量密切相关(P<0.05),而患者其他临床指标与其睡眠质量无明显相关性(P>0.05)。多因素逐步Logistic回归结果发现对AECOPD相关知识了解程度、心理因素(对死亡的焦虑和抑郁)是影响老年AECOPD住院患者睡眠质量的独立危险因素。结论老年AECOPD住院患者普遍存在睡眠质量问题,并受多方面因素的影响,其中对AECOPD相关知识了解程度、心理因素(对死亡的焦虑和抑郁)是影响老年AECOPD住院患者睡眠质量的独立危险因素。Objective To investigate the sleep quality of the patient with AECOPD, and to analysis the influencing factor in patient with AECOPD. Methods The retrospective analysis of clinical data of 95 patients with AECOPD were consecutively recruited from January of 2013 to January of 2014;Sleep qualityof the patient with AECOPD were investigated and assessed by PSQI.The influencing factors of sleep quality were analyzed.Results63 of 95 patients were found that they were took the sleep disorders in the investigation.The age, financial burden,treatment at night,the degree of recognition in AECOPD, psychological factor were correlated intimately with sleep quality（P〈0.05）;However,the else clinical target were not correlated significantly with sleep quality（P〉0.05）.Based on multivariant stepwise Logistic regression analysis,the degree of recognition in AECOPD and psychological factor were independent risk factor of sleep disorders in the patients with AECOPD.Conclusion Most hospitalized patients with AECOPD had sleep quality problem, and influenced by various factors.Among them,the degree of recognition in AECOPD and psychological factor were independent risk factor of sleep disorders in the patients with AECOPD.
