摘  要:目的 验证进行性核上性麻痹(PSP)患者脑部葡萄糖代谢特点在不同地区的2组患者中的可重复性,为多中心研究奠定基础.方法 对上海华山医院10例PSP确诊患者和10名健康对照者,以及北京协和医院13例PSP确诊患者和13名健康对照者行静息状态下18F-FDG PET检查.应用SPM分析法对不同地区的2组PET图像进行两样本t检验分析,分别获得2组PSP患者葡萄糖代谢异常脑区,并比较其变化特点的异同.结果 华山医院PSP患者与对照组相比,表现为中脑及双侧丘脑、双侧尾状核、双侧前额叶内侧和双侧前额叶(腹)外侧葡萄糖代谢的减低(t =5.26~ 11.00,P<0.001);协和医院PSP患者与对照组相比,表现为中脑及双侧丘脑、双侧尾状核、双侧前额叶内侧、双侧前额叶(腹)外侧和双侧角回葡萄糖代谢的减低(t=4.55~5.56,P<0.001).结论 不同地区的2组PSP患者均存在中脑及双侧丘脑、双侧尾状核、双侧前额叶内侧和双侧前额叶(腹)外侧葡萄糖代谢减低的表现,为18F-FDG PET显像应用于PSP鉴别诊断的多中心研究提供了客观依据.Objective To validate the reproducibility of abnormal cerebral glucose metabolic characteristics in patients with progressive supranuclear palsy (PSP) from different regions using 18F-FDG PET imaging.Methods Ten patients with PSP and 10 age-matched healthy controls from Shanghai Huashan Hospital (HI),as well as 13 patients with PSP and 13 age-matched healthy controls from Beijing Union Hospital (H2) were recruited to perform resting-state brain 18F-FDG PET imaging.The cerebral metabolic characteristics of patients were investigated using SPM with two-sample t test.Results The features of cerebral glucose metabolism by SPM analysis in PSP patients from H1 were demonstrated as decreased metabolism in midbrain,and bilaterally decreased metabolism in thalamus,caudate,medial prefrontal cortex and ventrolateral prefrontal cortex(t:5.26-11.00,all P〈0.001).Meanwhile,the similar hypometabolism in addition to that in the angular gyrus was found in patients with PSP from H2 (t:4.55-5.56,all P〈0.001).Conclusion Decreased metabolism exsists in the midbrain,thalamus,caudate,medial prefrontal cortex and ventrolateral prefrontal cortex of patients with PSP from different regions,which provide reliable evidence for the multi-center study about differential diagnosis of PSP based on 18F-FDG PET imaging.
