调脂治疗对改善早期胰岛素分泌的临床研究

摘　　要：目的探讨调脂治疗对2型糖尿病患者早期胰岛素分泌功能的改善作用。方法入组对象为新诊断的2型糖尿病患者118例,血脂正常者40例(T2DM-1组)及血脂异常者30例(T2DM-2组)仅接受单纯降糖治疗;血脂异常者48例(T2DM-3组)应用调脂联合降糖治疗,疗程共3月。治疗前后测血糖、血脂,行口服葡萄糖耐量试验(OGTT),计算OGTT中早期胰岛素分泌功能指数△I30/△G30。结果三组在研究终点时,糖化血红蛋白(HbA1c)均达标;T2DM-3组血脂达理想水平,而且在治疗中期早期胰岛素分泌功能指数△I30/△G30已经得到改善,至终点时与T2DM-1组恢复到相似水平,但T2DM-3组升高幅度明显大于T2DM-1组(P<0.05);T2DM-2组在治疗终点时血脂也得到不同程度的改善,但未达理想水平,终点时早期胰岛素分泌功能指数△I30/△G30与其他两组比较差异有统计学意义(P<0.05)。结论短期的调脂治疗能够改善伴脂代谢紊乱的初发2型糖尿病患者早期胰岛素分泌功能。Objective To study on the improvable effect of lipid-lowering treatment on early insulin secretion in T2 DM patients. Methods 118 newly diagnosed T2 DM patients were allocated to three groups：T2DM-1 group（n =40） without dyslipidemia and T2DM-2 group（n =30） with dyslipidemia only received antihyperglycemic treatment for three months,T2DM-3 group（n =48） with dyslipidemiareceived lipid-lowering and antihyperglycemic treatment for the same period.Fasting blood samples were collected for measuring gluclose, lipid profile, OGTTs andplasma insulin before and after treatment. The early insulin secretion indexes △I30/ △G30during OGTT were calculated. Results Hb A1 c of the three groups reached the target in the end. TG,LDL-C,TC and HDL-C reached the normal level in the T2DM-3 group.The early insulin secretion indexes △I30/ △G30had an increasing trend in the middle of treatment, and achieved a similar level with T2DM-1 group after treatment, but the increasing degree was higher in the T2DM-3 group than that of the T2DM-l group（P〈0.05）. TG, LDL-C, TC and HDL-C had different degrees of improvement in the T2DM-2 group in the end, but could not reach the target in the end, and there was statistical significant difference at the early insulin secretion indexes between the T2DM-2 group and the other two group（P 〈0.05）. Conclusion These results indicated that short-term lipid-lowering treatment improved the early insulin secretion of the newly diagnosed T2 DM patients with lipid metabolism disorder.
