育龄妇女宫内放置节育器不良反应分析

摘　　要：目的总结育龄妇女宫内放置节育器的不良反应,探讨解决措施。方法选择我院产科门诊手术室2012年1月至2014年10月600例宫内放置节育器的育龄期妇女作为观察对象,并对其临床资料进行回顾性分析,总结宫内放置节育器不良反应发生原因。结果宫内放置节育器的主要不良反应为节育器脱落(163例/27.2%),出血(107例/17.8%),生殖道感染(100例/16.7%),腰腹疼痛(87例/14.5%),月经过多(51例/8.5%),带器妊娠(14例/2.3%);所有患者经过对症处理后,症状均得到缓解。结论育龄期妇女宫内放置节育器后,可引发多种不良反应,导致因症取器发生率逐年上升。术前应根据子宫大小选择合适节育器,减少宫腔操作,告知育龄期妇女置器后应合理用药,并定期来院观察随访,能够有效减少置器不良反应发生率。Objective We were to summarize adverse reactions induced by intrauterine devices placed in fertile women and to explore solutions.Methods The fertile women who received intrauterine devices placement in department of gynecology in our hospital during June 2013 and October 2014, 600 cases, were chosen as research object, their clinical data were analyzed retrospectively, summarized the reasons which caused adverse reactions.Results The main adverse reactions induced by intrauterine devices place were the synthetic fall off （163, 27.2%）, bleeding （107, 17.8%）, reproductive tract infections （100, 16.7%）, waist and abdomen pain （87, 14.5%）, menorrhagia （51, 8.5%）, pregnancy with device （14, 2.3%）.After symptomatic treatment, all symptoms were alleviated.Conclusions The placement of intrauterine devices can cause a variety of adverse reactions.The incidence of intrauterine devices taken is rising year by year.Choose intrauterine devices according to uterine cavity size, reduce operation in uterine cavity, rational administration and regularly hospital visit can effectively reduce incidence of adverse reactions.
