三种黄体支持方案对激素替代周期冻融胚胎移植妊娠结局的影响

摘　　要：目的探讨3种黄体支持方案对激素替代周期行冻融胚胎移植患者妊娠结局的影响。方法回顾性分析2011年10月至2013年10月在我院生殖中心接受激素替代周期冻融胚胎移植的219个周期,根据不同的黄体支持方案分为3组,A组使用黄体酮针(91例)、B组联合使用黄体酮胶囊和黄体酮软胶囊(68例)、C组联合使用地屈孕酮片和黄体酮软胶囊(60例)。比较并分析3种黄体支持方案患者的优胚数、冻胚数、胚胎冷冻时间、移植胚胎数、胚胎复苏率、移植日子宫内膜厚度、胚胎种植率、临床妊娠率和早期流产率。结果 3组患者优胚数、冻胚数、胚胎冷冻时间、移植胚胎数、胚胎复苏率、移植日子宫内膜厚度相比较,差异均无统计学意义(P均>0.05);A、B、C组患者的胚胎种植率分别为20.0%、19.8%、21.7%,临床妊娠率分别为35.2%、35.3%、38.3%,早期流产率分别为9.4%、12.5%、8.7%,3组相比较,差异均无统计学意义(P均>0.05)。结论黄体酮针、黄体酮胶囊联合黄体酮软胶囊、地屈孕酮片联合黄体酮软胶囊3种黄体支持方案在激素替代周期行冻融胚胎移植患者中可取得相似的临床效果。Objective. To explore the effects of three different luteal phase support protocols on clinical outcomes of frozen-thawed embryo transfer（FET）in hormone replacement treatment（HRT）cycles. Methods： The data of 219 HRT-FET cycles were retrospectively analyzed in our center from October 2011 to October 2013. The cycles were divided into three groups： the patients received intramuscular progesterone in group A; the patients received a combination of vaginal micronized progesterone and oral progesterone in group B; and the patients received vaginal micronized progesterone and dydrogesterone in group C. The implantation rate, clinical pregnancy rate and early miscarriage rate were compared among the three groups. Results： The implantation rate was 20.0%,19.8% and 21.7% ,clinical pregnancy rate 35.2%,35.3% and 38.3% ,early miscarriage rate 9.40%,12.5% and 8.7% respectively in the three groups. There was no significant difference in the clinical outcomes among the three groups（P〉0.05）. Conclusions： In the HRT-FET cycles, the similar clinical outcomes can be obtained when the luteal phase is supported by intramuscular progesterone,vaginal micronized progesterone and oral progesterone, or vaginal micronized progesterone and dydrogesterone.
