摘 要:目的探讨心室射血分数自动测量(Auto EF)法评价心肌梗死患者左室整体收缩功能的可行性和重复性。方法选取我院心肌梗死患者(心肌梗死组)和健康体检者(健康对照组)各35例,应用Auto EF法测量左室舒张末期容积(LVEDV)、收缩末期容积(LVESV)及射血分数(LVEF);并与传统双平面Simpson’s法比较,分析其相关性;随机选取20例心肌梗死患者行Auto EF重复性研究。结果 70例研究对象中,68例(97%)完成Auto EF测量,其中43例(63%)为完全自动,25例(37%)需手动校正;Auto EF测量时间为(59.28±13.13)s,少于Simpson’s法[(96.67±8.26)s],差异有统计学意义(P<0.01);68例总体Auto EF与双平面Simpson’s法测量LVEDV、LVESV、LVEF均有高度相关性(r分别为0.947、0.971、0.974,P<0.01);心肌梗死组和健康对照组中,两法所测LVEDV、LVESV和LVEF差异均无统计学意义,且有良好相关性,但心肌梗死组相关系数均较健康对照组偏低;在观察者自身和观察者间,Auto EF法测定心肌梗死患者LVEF均有良好的一致性。结论 Auto EF法能够简便、及时和快速测量左室容积和LVEF,且经验依赖性较小。Objective To explore the feasibility and reproducibility of assessing left ventricular(LV) global systolic function of the patients with myocardial infarction by a novel automated ejection fraction(AutoEF). Methods Thirty-five patients with myocardial infarction and 35 healthy subjects were included. LV end diastolic volume(LVEDV),LV end systolic volume (LVESV) and LV ejection fraction (LVEF) measured by AutoEF, the results were compared with those of biplane Simpson's method. The correlation between two methods was observed. 20 patients with myocardial infarction were randomly selected to study the reproducibility. Results AutoEF was performed in 68 patients (97%),43(63%) of them were completely automated and 25(37%) required manual editing. Automatic measurement required less time than biplane Simpson's method [ (59.28±13.13)s vs.(96.67±8.26)s,P〈0.01 ]. The series LV parameters of the 68 cases, which including myocardial infarction group and healthy control group had strong correlation in LVEDV,LVESV and LVEF between two methods(r were 0.947, 0.971,0.974,respectively, P〈0.01).There were no statistical differences between AutoEF and biplane Simpson's method for LVEDV,LVESV and LVEF in two groups. LVEDV,LVESV and LVEF measured by AutoEF had good correlation with those measured by biplane Simpson' s method, but the correlation coefficients of myocardial infarction group were lower than that of healthy control group. LVEF were measured by intraobserver and interobserver using AutoEF which had a good consistency. Conclusion AutoEF,a convenient method,can measure LV volume and ejection fraction promptly and rapidly with less preconception.
