国产微创椎弓根钉系统治疗胸腰椎骨折的近中期疗效评估  被引量:2

Short-and Mid-term Effects of Domestic Minimally-invasive Percutaneous Screw System for the Treatment of Thoracolumbar Fractures

摘  要:目的评价国产微创经皮空心椎弓根钉系统内固定治疗无神经损伤胸腰椎骨折的临床疗效。方法选择2011年1月-2012年4月无明显神经损伤的不稳定胸腰椎骨折患者60例,分为A组(国产微创经皮空心椎弓根钉内固定)和B组(进口微创经皮空心椎弓根钉内固定)。比较两组围手术期参数、疼痛视觉模拟评分(VAS)、影像学指标及疗效情况(改良Mac Nab疗效判定标准)。结果所有患者均获得随访,随访时间为6-18个月,平均(12.2±3.0)个月。两组术后腰背部疼痛较术前均有明显减轻,术后功能恢复良好。两组椎弓根钉置入准确率比较差异无统计学意义(P〉0.05),术后伤椎椎体前缘高度与Cobb角较术前均有显著改善(P〈0.05),但两组手术前后比较差异均无统计学意义(P〉0.05)。结论国产微创经皮空心椎弓根钉治疗无神经损伤胸腰椎骨折具有价格低廉,术后功能恢复好,其复位固定效果与进口经皮Sextant椎弓根钉一致,值得临床推广应用。Objective To evaluate the clinical efficacy of domestic minimally-invasive percutaneous screw system for thoracolumbar fractures without neurological damage.Methods Sixty patients suffering from unstable thoracolumbar fractures without obvious neurologic deficits treated from January 2011 to April 2012 were studied retrospectively.The patients were divided into two groups:group A(domestic minimally-invasive percutaneous screw system) and group B(imported minimally-invasive percutaneous screw system).Perioperative parameter,pre-and postoperative imaging indexes,visual analog scale(VAS) and modified MacNab evaluation standard were studied for comparison.Results All the patients were followed up from 6 to 18 months with an average of(12.2±3.0) months.The Cobb's angle and anterior height of the fracture vertebral body changed significantly in each group(P〈0.05).There was no significant difference in incision size,surgical time,postoperative improvement of Cobb's angle,anterior height of the fracture vertebral body and accuracy of pedicle screw placement between the two groups(P〉0.05).Conclusion Domestic minimally-invasive percutaneous screw system is reliable with minimal invasion,which is comparable to imported minimally-invasive percutaneous screw system.
