摘　　要：目的:探讨CD4+LAP+T细胞在慢性心力衰竭(CHF)患者中变化及意义。方法:本研究共纳入85例研究对象,其中CHF患者55例(病例组):缺血性心脏病(IHD)30例,非缺血性心脏病(NIHD)25例;对照组30例。采用流式细胞术分别测定病例组和对照组外周血中CD4+LAP+T细胞在CD4+T细胞中的比例;采用夹心酶联免疫法(ELISA)测定转化生长因子(TGF)-β1、白细胞介素(IL)-10的浓度。结果:病例组外周血中CD4+LAP+T/CD4+T细胞比例显著低于对照组[(2.23±0.76)%∶(3.66±0.84)%,P<0.05];病例组血浆中TGF-β1的水平显著低于对照组[(256.52±78.74)pg/ml∶(386.83±85.27)pg/ml,P<0.05],病例组血浆中IL-10水平显著低于对照组[(16.82±5.21)pg/ml∶(25.71±5.81)pg/ml,P<0.05]。相关性分析提示CD4+LAP+T细胞比例与血浆TGF-β1、IL-10、左室射血分数呈显著正相关,与左室舒张末期内径、C-反应蛋白水平呈显著负相关。结论:CD4+LAP+T细胞比例在CHF患者中显著减少,相关细胞因子表达下调,预示CD4+LAP+T细胞减少在CHF的进展中发挥促进作用。Objective：To investigate the CD4＋LAP＋T cells frequency in chronic heart failure（CHF）by flow cytometry,and discuss the functions of CD4＋LAP＋T cells in the patients suffered from CHF.Method：The 55 CHF patients and 30 controls were obtained in our research,the frequencies of CD4＋LAP＋T cells in CD4＋T cells in the peripheral blood were investigated by flow cytometry in both the CHF group and the control group.Then the relating plasma cytokine（TGF-β1,IL-10）concentration were detected by ELISA.Result：The frequencies of the CD4＋LAP＋T cells in the CHD group were significantly lower than the control group[（2.23±0.76）% vs（3.66±0.84）%,P〈0.05].Both plasma TGF-β1and IL-10 concentration were significantly lower in CHF group than control group[（256.52±78.74）pg/ml vs（386.83±85.27）pg/ml and（16.82±5.21）pg/ml vs（25.71±5.81）pg/ml,P〈0.05 ];Results also showed that the frequencies of CD4＋LAP＋T cells were positively related to plasma TGF-β1,IL-10 concentration and left ejection fraction,but were inversely related to the C-reactive protein and left ventricular diastolic diameter.Conclusion：CD4＋LAP＋T cells are impaired in patients with CHF,and play an important role in the development of CHF.
