摘 要:目的探讨糖耐量正常的代谢综合征人群的血糖特点。方法2009年6至12月中山大学附属第三医院40例健康体检者经口服葡萄糖耐量试验(OGTT)诊断均为正常糖耐量(NGT),依据代谢综合征诊断标准分为代谢综合征组(MetS组,12例)和非代谢综合征组(N-MetS组,28例)并进行动态血糖监测。本研究通过中山大学附属第三医院伦理委员会批准。结果MetS组与N-MetS组空腹血糖、OGTT 2h血糖和糖化血红蛋白差异无统计学意义。MetS组血糖波动指标值较N.MetS组升高,其中日内平均血糖漂移幅度(MAGE)差异有统计学意义[(2.12±0.92)mmol/L与(1.50±0.68)mmol/L,P=0.02]。Logistic回归分析显示MAGE与代谢综合征具有相关性(OR=2.74,95%CI:1.08-6.97,P=0.03)。结论与不合并代谢综合征的NGT者相比,合并代谢综合征的NGT者血糖波动幅度升高,MAGE可能是描述该人群血糖代谢状况更为合适的指标。Objective To explore the characteristics of glycemic variability in normal glucose tolerance (NGT) subjects with metabolic syndrome (MetS). Methods A total of 40 NGT subjects diagnosed by oral glucose tolerance test (OGTT) were divided into two groups of metabolic syndrome (MetS) or without MetS (Non-MetS) according to the National Cholesterol Education Program-Adult Treatment Panel Ⅲ ( NCEP-ATP Ⅲ ) criteria. Fasting blood glucose ( FBG), OGTT 2 h blood glucose (2 h BG), hemoglobin Alc(HbAlc) and glycemic variability were assessed. Glycemic variability was assessed by calculating standard deviation of MBG (SDBG), mean amplitude of glycemic excursions ( MAGE), largest amplitude of glycemic excursions (LAGE) and means of daily differences (MODD) from the 72 h data of continuous glucose monitoring system (CGMS) in real life. The time of diurnal glycemic variability was also calculated. Results No significant inter-group difference existed in FBG, 2 h BG or HbAlc. Glycemic variability was higher in MetS than that in Non-MetS group. But only MAGE (2. 12 ± 0.92 vs 1.50 ± 0. 68 mmol/L, P = 0. 02) showed significant inter-group difference. Logistic regression analysis revealed that MAGE was significantly correlated with MetS ( OR = 2. 74, 95% CI: 1.08 - 6. 97, P = 0. 03) and independently from FBG, PBG, HbAlc and other glycemic variability indices. Conclusions Glycemic variability increases in NGT subjects with MetS. And MAGE may better describe the population's glucose metabolism.
