摘 要:目的探讨慢性乙型肝炎(CHB)患者体内转录因子Foxo3a表达水平与外周血CD8^+T细胞亚群凋亡的关系。方法选取就诊同济大学附属第十人民医院的42例CHB患者及24例正常对照,分离外周血PBMC细胞,流式细胞仪检测CD8T细胞内的细胞因子,采用AnnexinV.FITC,PI及抗CD8抗体三标法检测CD8^+T细胞的凋亡比率,采用流式APC,FITC抗CD8及二抗检测CD8^+T细胞Foxo3a表达水平,定量PCR检测PBMC细胞Foxo3a表达水平,Western印迹法检测Foxo3a蛋白水平。对CD8^+T细胞凋亡率、细胞内Foxo3a表达水平以及Foxo3a水平与CD8^+T细胞凋亡的关系等进行分析。结果CHB患者CD8^+T细胞亚群凋亡比例[(21.18±2.48)%]高于正常对照组[(7.52±0.42)%](t=18.34,P〈0.05),CD8^+T细胞Foxo3a水平为(0.55±0.09),高于正常对照组的(0.14±0.03)(t=4.54,P〈0.05)。多元回归分析显示:CD8^+T细胞凋亡率与Foxo3a表达水平有线性回归关系(F=141.451,P〈O.01)。结论CHB患者外周血CD8呵细胞凋亡增强及功能缺陷使细胞免疫功能减弱可能与患者体内Foxo3a表达水平相关。Objective To evaluated the relationship between the expression level of a forkhead transcrip- tion factor Foxo3a and the apoptosis of CD8^+T lymphocytes in chronic hepatitis B patients from Tenth People's Hospital of Tongji University. Methods There were 42 cases of patients with chronic hepatitis B and 24 cases of normal control group. PBMC were derived from peripheral blood cells. The levels of intracellular cytokine of CD8^+T cells were analyzed by flow cytometry. Cell apoptosis ratio was detected by Annexin V-FITC, Propidium Iodide (PI) and CD8 antibody three standard method. The expression of Foxo3a in CDSCF cells was analyzing by flow cytometry APC, FITC CD8 antibody and second antibodies, while the expression of Foxo3a in PBMC was detected by RT-PCR , and the protein level of Foxo3a was detected by western blot. The cytokines, cell apoptosis ratio and Foxo3a expression level in CD8CF cells were analyzed. Meanwhile, the apoptosis ratio of CD8^+T, the relationships among the expression level of Foxo3a, the level of Foxo3a and CD8^+T cell apoptosis were analyzed. Results The CD8CF apoptosis ratio [(21.18±2.48)%] in patients with chronic hepatitis B was higher than the normal control group[(7.52±0.42)%](t=18.34,P〈0.05). The expression of Foxo3a in patients with chronic hepatitis B was (0.55±0.99) and higher than the normal control group (0.14±0.03), of which the difference had statistical significance (t=4.54,P〈0.05). Multiple regression analysis showed that the linear regression relation between the expression of CDS+T apoptosis ratio and the level of Foxo3a(F=141.451,P〈0.01 ). Conclusions The increasing apoptosis and defective function of CDS+T lymphocytes in patients with chronic hepatitis B make the cell immune function to decrease, which is probably associated with the expression of Foxo3a.
