摘　　要：目的:探讨冠状动脉粥样硬化性心脏病(简称冠心病)临床心力衰竭(阶段C)患者中医证候演变规律及用药特点。方法:运用信息采集表收集北京中医药大学东直门医院2010年1月至2013年12月就诊的151例冠心病临床心力衰竭患者病历资料,回顾分析临床心力衰竭患者(阶段C)中医证候演变规律及益气活血中成药、西药应用情况。结果:冠心病临床心力衰竭(阶段C)患者中医证型均以气虚血瘀证为主,其进展与中、西药物的联合应用具有一定相关性(P=0.012)。结论:气虚血瘀是冠心病慢性心力衰竭(CHF)临床心衰(阶段C)发展的基本病理环节,在西药常规治疗基础上联合应用益气活血药物,可在一定程度上延缓临床心力衰竭病程的进展,提示益气活血法对冠心病CHF临床前心力衰竭(阶段B)患者早期应用有积极意义。Objective： To investigate the TCM syndrome and treatment evolving rules and medicating characters for heart failure（stage C） induced by coronary atherosclerotic heart disease（CHD for short） in clinic. Methods：The medical records of 151 CHD patients＇ with heart failure from January 2010 to December 2013 in Dongzhimen Hospital of Beijing University of Chinese Medicine were gathered by information collection table. The evolution rules of TCM syndromes and the medication by Qi-supplementing and blood-activating medicine as well as western medicine for CHD patients with heart failure（stage C） were reviewed and analyzed. Results： The TCM syndrome of heart failure（stage C） induced by CHD mainly was qi deficiency and blood stasis syndrome and its development had certain correlation with the combination of western medicine（P=0.012）. Conclusion： Qi deficiency and blood stasis are the basic pathological factors for the development of heart failure（stage C） induced by CHD. The conventional treatment by western medicine with the combination of qi-supplementing and blood-activating medicine can postpone the development of heart failure in a certain degree, which indicates that the early application of supplementing Qi and activating blood circulation method has positive meaning for the treatment of heart failure（stage B） due to CHD in clinic.
