下床时间对椎间融合器联合内固定治疗的腰椎退行性疾病患者术后康复的影响

摘　　要：目的探讨椎间融合器联合椎弓根钉内固定治疗腰椎退行性疾病患者术后早期下床对腰椎功能恢复及腰腿痛的影响,为患者在临床中进行康复指导提供理论和实践依据。方法选择腰椎退行性疾病行后路椎间融合器联合椎弓根螺钉内固定患者80例,随机分成观察组和对照组各40例,观察组术后3 d佩戴腰部护具下床活动,对照组术后14 d佩戴腰部护具下床活动。比较两组患者术后1、3和6个月的Oswestry功能障碍指数(ODI)及腰腿痛的视觉模拟疼痛评分(VAS)。结果观察组的ODI指数在术后1和3个月明显低于对照组,差异有统计学意义(P<0.05);术后6个月两组比较差异无统计学意义(P>0.05);两组患者的腰腿痛VAS评分在术后1、3和6个月比较,观察组明显低于对照组,差异有统计学意义(P<0.05)。结论腰椎退行性疾病患者行椎间融合器联合椎弓根钉内固定手术术后3d下床可以促进腰椎功能的恢复,减轻腰腿痛,提升手术效果,促进患者康复,是可行性的。[ Objective ] To explore the effect of ambulation time on the progosis of patients with lumbar degenerative conditions treated with intervertebral fusion cage combined with internal fixation with pedicle screws, and provide theoretical and practical basis for the rehabilitation. [ Methods ] Eighty patients with lumbar degenerative conditions treated with intervertebral fusion cage combined with internal fixation with pedicle screws were randomly divided into observed group （40 cases） and control group （40 cases）. Patients in observed group wearing the waist protecting equipment left the bed 3 days after operation, and patients in control group wearing the waist protecting equipment left the bed 14 days after operation. The oswestry disability index （ODI） and visual analogue scale （VAS） 1, 3, 6 months after operation between two groups were compared randomly. [ Results ] The ODI scores in observed group 1, 3 months after operation were significantly lower than those in the control group, and there was significant difference between them （P 〈 0.05）. There was no significant difference between them after 6 months （P 〉 0.05）. The VAS scores in observed group 1, 3, 6 months after operation were significantly lower than those in the control group, and there was significant difference between them （P 〈 0.05）. [ Conclusions ] It＇s available for patients with lumbar degenerative conditions treated with intervertebral fusion cage combined with internal fixation with pedicle screws to leave bed 3 days after operation, which can improve the lumbar degenerative conditions and relieve the postoperative pain of waist and leg with better prognosis.
