腰椎术后首次下床预防体位性低血压规范流程的实施及效果评价

摘　　要：目的探讨腰椎术后首次下床预防体位性低血压规范化流程的实施方法及效果。方法选择我科2014年1-6月腰椎手术患者共264例,随机分为对照组(未实施规范化流程)和观察组(实施规范化流程)各132例,比较两组患者体位性低血压(有症状者)的发生率。结果按AAS和AAN标准:对照组有50例患者在术后首次下床时出现不同症状、不同程度的体位性低血压,发生率为37.9%;观察组有11例患者出现体位性低血压,发生率为8.3%。结论腰椎术后首次下床规范化流程的实施可预防体位性低血压的发生,避免患者跌倒、骨折、再骨折、切口撕裂、腰椎内固定松动或钉棒断裂骨折或再骨折等护理不良事件的发生,确保患者安全,从而减少患者投诉或医疗纠纷。Objective To study the implementation and effect for the standardized process of prevention orthostat‐ic hypotension for patients after lumbar spine surgery first time out of bed .Method 264 patients undergoing lumbar surgery in our department from Jurnary 2014 to to June 2014 was selected and randomly divided into control group （not implementation of standardized processes） and observation group （the implementation of standardized proces‐ses） ,132 cases in each group .The incidence of orthostatic hypotension （with symptoms） was measured and com‐pared between the two groups of patients .Result By AAS and AAN criteria：50 patients in the control group was with different symptoms after surgery for the first time out of bed ,and with varying degrees of orthostatic hypoten‐sion ,the rate was 37 .9% .11 patients in observation group had orthostatic hypotension ,the rate was 8 .3% .Con‐clusion Implementation of the standardized process of for patients after lumbar spine surgery first time out of bed will helpful to prevent orthostatic hypotension ,and to avoid falls ,fractures or re‐fracture adverse events during nursing .
