持续正压通气治疗对29例OSAHS患者血脂、血糖的影响分析

摘　　要：目的：探讨重度阻塞性睡眠呼吸暂停低通气综合征（OSAHS）患者持续正压通气治疗前后血脂和空腹血糖（FBG）的变化。方法：29例成年重度OSAHS患者经CPAP治疗前及治疗后（4周、24周）测体重、血脂水平、FBG等各项指标，并进行统计学分析。结果：OSAHS患者经Auto-CPAP治疗后血清总胆固醇（TC）、甘油三酯（TG）、低密度脂蛋白（LDL—CH）、FBG呈现下降趋势，而高密度脂蛋白（HDL—CH）出现升高趋势，这种升高和下降的趋势在治疗24周后差异有统计学意义（P〈0．05），但在治疗4周前血脂、血糖水平变化差异无统计学意义（P〉0．05）。经24周治疗后体重指数（BMI）明显下降，从治疗前（30．2±3．2）kg／m^2下降到（27．8±6．3）kg／m^2（P〈0．05）。结论：使用CPAP治疗24周后可以改善重度OSAHS患者的BMI、血脂、血糖；减轻体重以及纠正睡眠呼吸紊乱在改善OSAHS患者的糖脂代谢异常方面有着重要意义。Objective Study of severe obstructive sleep apnea hypopnea syndrom . （OSAHS） patients before and after treatment of continuous positive airway pressure changes of blood lipid and fasting blood glucose （FBG）. Methods 29 cases of adult patients with severe OSAS CPAP treatment before and after treatment （4 weeks and 24 weeks） to measure weight, lipid levels, FBG, such as the indicators, and statistical analysis. Results OSAHS patients after treatment with Auto - CPAP serum total cholesterol （TC）, triglycerides （TG）, low density lipoprotein （LDL - CH） presents downtrend, FBG, and high-density lipoprotein （HDL - CH） appeared rising trend, the trend of the rise and fall of the 24 weeks after treatment was statistically significant difference （P 〈 0.05）, 24 weeks after the treatment of body mass index （BMI） significantly decreased, from before the treatment （30.2 ＋ / - 3.2） kg/m^2 dropped to （27.8 ＋/- 6.3） kg/m^2 （P 〈 0.05）. Conclusions Using CPAP treatment can improve the severe OSAHS patients after 24 weeks of BMI, blood lipid, blood sugar; Weight loss and correct sleep-related breathing disorder in improving the glucolipid metabolic abnormalities in patients with OSAHS has important significance.
