急诊胃大部分切除术治疗胃十二指肠溃疡穿孔的临床效果及安全性分析

摘　　要：目的：探讨胃大部分切除治疗胃十二指肠溃疡穿孔的疗效以及安全性。方法：选取我院2012年6月-2014年6月收治的胃十二指肠溃疡穿孔的患者38例，随机分为观察组与对照组，每组各19例，观察组患者应用胃大部分切除术治疗，对照组患者给予胃穿孔修补术治疗，对比两组患者手术时间与术中出血量以及并发症的几率。结果：观察组患者手术时间以及术中出血量较高，观察组患者出现并发症明显低于对照组，两组对比差异显著（P〈0．05），有统计学意义。结论：采用胃大部分切除术治疗胃十二指肠溃疡穿孔其远期疗效较高，并发症较少，但由于该方式术中出血量较多，且手术时间较长，因此必须根据患者的实际情况以及适应证进行选择。Objective To probe into the clinical effects and safety of subtotal gastectomy in treatment of patients with acute perforation of gastro duodenal ulcers. Methods 38 patients who suffered from the gastric and duodenal ulcer perforation and who accepted treatments in our hospital from June, 2012 to June, 2014 were taken as the research objects, and these patients were randomly divided into the control group and the observation group, with 19 patients in each group. In the observation group, they were treated with Subtotal gastrectomy while in the control group, they were treated with of repairable gastric perforation. Then, the operation time and intraoperative bleeding volume and the probability of complications of these two groups of patients were compared. Results The operation time and intraoperative bleeding volume and the probability in the observation group were higher, while the probability of complications in the observation group were lower than those in the control group. In addition, the significant differences of two groups had the statistical significance （P〈0.05）. Conclusion Using partial subtotal gastrectomy in treatment of gastric and duodenal ulcer perforation has the long-term effect, while complications were fewer. However, because of the way in operation has amount of more bleeding in addition, the operation time is longer, therefore the oatients must be selected according to the actual situation and indications.
