Treatment of thoracolumbar burst fracture with unilateral transpedicular screw fixation combined with bone grafting in injured vertebral body through pedicle passage on the opposite side

摘　　要：目的探讨经伤椎单侧椎弓根固定及植骨治疗胸腰椎骨折的临床疗效。方法对23例胸腰椎骨折患者应用椎弓根钉棒系统治疗,经伤椎一侧椎弓根螺钉固定,经另一侧椎弓根通道植骨。X线片评价术前、术后1周及末次随访时的伤椎前缘高度、后凸Cobb角。结果患者均获随访,时间8~24（10±2）个月,无内固定物失效,胸腰椎高度和后凸矫正度无明显丢失,骨折愈合良好。结论经伤椎单侧椎弓根固定及植骨治疗胸腰椎骨折可以减少内固定失败和矫正度的丢失,操作简单,临床效果满意。Objective To investigate the clinical effect of treatment of thoracolumbar burst fracture by pedicle screw fixation through single pedicle passage of injured vertebra combined with bone graft through the opposite side. Methods 23 cases of thoracolumbar fracture were treated by pedicle screw fixation,the injured vertebra was fixed with pedicle screw at one side,and the other side got transpedicular bone grafting simultaneously. All data of the patients including the anterior marginal height of the injured vertebra and the Cobb＇ s angle were evaluated in preoperation,one week after operation and the last follow-up. Results All cases were followed up for 8 ~ 24（10 ± 2） months,the internal fixation instrument failure and lost of correction of the anterior marginal height of the injured vertebra and the Cobb＇s angle were not observed obviously,all the patients got fracture healing. Conclusions By placing pedicle screw in one side combined with transpedicular bone grafting in the other side at the fractured vertebra is an effective and simple method in the treatment of thoracolumbar fractures,because it is helpful in preventing failure of fixation,correction loss and vertebra deformity.
