骶尾骨前网片固定悬吊术联合电刺激生物反馈疗法对老年女性盆底功能康复的影响

摘　　要：目的探讨骶尾骨前网片固定悬吊术联合电刺激生物反馈疗法对老年盆腔脏器脱垂(POP)术后盆底功能康复的影响。方法将96例老年POP患者随机分为对照组与观察组,每组48例。2组均行骶尾骨前网片固定悬吊术治疗;观察组术后1个月采用电刺激生物反馈疗法。比较2组术前与术后6个月压力性尿失禁(SUI)及盆底肌力情况。结果 2组术后均能有效减少SUI发生,观察组术后6个月SUI改善较对照组更为显著(χ2=7.470 1,P=0.023 9)。对照组术后盆底肌力无明显改善(χ2=1.303 2,P=0.860 8),观察组能明显增强盆底肌力,治疗前后比较差异有统计学意义(χ2=37.052 0,P=0.0000);术后6个月2组盆底肌力改善情况比较,差异有统计学意义(χ2=33.643 1,P=0.0000)。结论骶尾骨前网片固定悬吊术联合电刺激生物反馈疗法能从结构和功能上改善老年POP患者的盆底功能。ABSTRACT:Objective To investigate the effect of anterior sacrococcygeal mesh suspension sur-gery combined with electrical stimulation biofeedback therapy on pelvic floor rehabilitation in eld-erly women with pelvic organ prolapse(POP).Methods Ninety-six elderly women with POP were randomly treated with anterior sacrococcygeal mesh suspension surgery(control group,n=48)or in combination with electrical stimulation biofeedback therapy 1 month after surgery(ob-servation group,n=48).Stress urinary incontinence and pelvic floor muscle strength were evalua-ted before and 6 months after surgery.Results The SUI was improved in both groups,and the improvement in observation group was more obvious than that in control group(χ2 =7.470 1,P =0.023 9).After treatment,the pelvic floor muscle strength was significantly improved in observa-tion group(χ2 =37.052 0,P =0.000 0),but not in control group(χ2 = 1.303 2,P = 0.860 8). There was a significant difference in the improvement in pelvic floor muscle strength between the two groups 6 months after surgery(χ2 =33.643 1,P =0.000 0).Conclusion Anterior sacrococcy-geal mesh suspension surgery combined with electrical stimulation biofeedback therapy can im-prove the structure and function of pelvic floor in elderly women with POP.
