摘  要:目的探讨气钡造影、MSCT增强扫描在食管胃结合部腺癌(AEG)术前评价中的应用价值。方法回顾性分析62例2009年1月至2014年4月在我院手术的AEG患者的气钡造影、MSCT影像资料,并与术后病理对照。结果 62例中气钡造影、MSCT扫描肿瘤检出率分别为100%、96.8%,Borrmann分型准确率分别为98.4%、98.3%,Siewert分型准确率均为100%,食管受侵检出率分别为93.5%、93.3%;影像测得食管浸润长度值与显微镜下测得长度值差异明显(P<0.05),MSCT对TN分期及浆膜受侵的准确率分别为83.3%、56.7%、96.7%。结论气钡造影及MSCT三期增强扫描可用于AEG术前肿瘤检出、食管受侵判断及Borrmann分型、Siewert分型,准确性高;CT对T分期及浆膜受侵判断准确性高。Objective To investigate the value of double-contrast barium meal and MSCT three-phase enhanced scan in the evaluation of adenocarcinoma of the gastroesophageal junction (AEG) preoperatively. Methods The double-contrast barium meal and MSCT enhanced scan findings of 62 patients with AEG from January 2009 to April 2014 were analyzed retrospectively, and the comparison was made with their postopera- tive pathologic results. Result The detection rate of double-contrast barium meal and MSCT enhanced scan was 100% and 96.8%, the accuracy of Borrmann classification was 98.4% and 98.3%, the accuracy of Siewert classification was 100% and 100%, the detection rate of esophagus infiltrating was 93.5% and 93.3%. There was significantly difference in the esophagus infiltrating length between imaging study and microscopy (P 〈 0.05). The accuracy of MSCT in the evaluation of TN staging and serosal invasion were 83.3%, 56.7% and 96.7%. Condusion Double-contrast barium meal and MSCT three-phase enhanced scan could be used for the preoperative detection, the judgment of esophageal infiltration length, Borrmann classification, and Siewert classification of AEG. The accuracy of MSCT in the evaluation of TN staging and serosal invasion degree was relatively higher.
