摘  要:目的 探讨亚甲基四氢叶酸还原酶(MTHFR)基因C677T位点单核苷酸多态性(SNP)与高尿酸血症(HUM)及痛风易感性的关联情况.方法 通过中国生物医学文献数据库、中国期刊网全文数据库、万方、维普、PubMed、Cochrane Library 、OvidSP、Wiley Online Librau、EBSCO、Elsevier Science Direct、Springer Link、Google scholar数据库检索MTHFR基因C677T位点SNP 1j HUM、痛风相关的病例-对照研究,利用RevMan 5.3和Stata 12.0软件,根据异质性大小选用随机效应或固定效应模型对各研究数据进行综合统计分析 结果 共纳入10篇HUM文献,包括1 899例患者和5 403例对照;3篇痛风文献,包括209例患者和194例对照HUM的Meta分析结果提示携有CC基因型[比值比(OR)=0.42,95%CI(0.29,0.63),P<0.01]和C等位基因[OR=0.54,95%CI (0.42,0.71),P<0.01]的人群罹患HUM的风险降低,携有TT基因型[OR=1.55,95%CI (1.32,1.83),P<0.01]和T等位基因[OR =1.84,95%CI (1.38,2.45),P<0.01]的人群罹患HUM的风险增高痛风的Meta分析结果提示携有CC基因型[OR=0.32,95%CI(0.15,0.68),P=0.003]和C等位基因[OR=0.35,95%CI (0.17,0.70),P=0.003]的人群罹患痛风的风险降低,携有TT基因型[OR =2.92,95%CI(1.74,4.92),P<0.01]和T等位基因[OR=2.69,95%CI(1.50,4.84),P=0.000 9]的人群罹患痛风的风险增高.结论 MTHFR基因C677T位点SNP与HUM、痛风易感性之间有关联性.Objective To explore the association between C677T polymorphism of methylenetetrahydrofolate reductase (MTHFR) gene and susceptibility to hyperuricemia and gout.Methods The case-control studies from Chinese Biomedical Database,Chinese National Knowledge Infrastructure,Wanfang,Weipu,PubMed,Cochrane Library,OvidSP,Wiley Online Library,EBSCO,Elsevier Science Direct,Springer Link and Google scholar databases for the association of C677T polymorphism of MTHFR gene with hyperuricemia and gout.The pooled odds ratios (OR) with 95% confidence intervals (CI) were appropriately derived from randomeffects or fixed-effects models to assess the association strength by RevMan 5.3 and Stata 12.0 software.Results Ten references enrolled 1 899 hyperuricemia patients and 5 403 controls,and 3 references enrolled 209 gout patients and 194 controls in total.The meta analysis showed that carriers of genotype CC [OR=0.42,95%CI (0.29,0.63),P〈0.01] and allele C [OR=0.54,95%CI (0.42,0.71),P〈0.01] had lower risk of hyperuricemia;genotype TT [OR=1.55,95%CI (1.32,1.83),P〈0.01] and allele T [OR=1.84,95%CI (1.38,2.45),P〈0.01] had higher risk of hyperuricemia,carriers of genotype CC [OR=0.32,95%CI (0.15,0.68),P=0.003] and allele C [OR=0.35,95%CI (0.17,0.70),P=0.003] hadlower risk of gout;genotype TT [OR=2.92,95%CI (1.74,4.92),P〈0.01] allele T [OR=2.69,95%CI(1.50,4.84),P=0.0009] had higher risk of gout.Conclusion The meta analysis reveals that C677T polymorphism of MTHFR gene is associated with the susceptibility to hyperuricemia and gout.
