磁共振成像评价脑内动脉狭窄与脑血流量关系的应用价值

摘  要:目的:探讨时间飞跃法磁共振血管成像(Time of flight-magnetic resonance artery,TOF-MRA)及磁共振动脉自旋标记灌注成像(Arterial spin labeling,ASL)在脑缺血病变患者脑内动脉狭窄与脑血流量(Cerebral blood flow,CBF)关系的应用价值。方法:将2013年9月至2014年8月的12例正常志愿者及临床诊断为脑缺血的52例患者在颅脑常规平扫基础上行采集TOF-MRA及ASL三维采集扫描,分析脑缺血患者动脉狭窄程度与脑血流量的关系。结果:正常志愿者TOF-MRA均正常,ASL示双侧对称部位CBF比值为0.92-1.09;检出所有患者脑动脉不同程度狭窄,正常志愿者TOF-MRA均正常,ASL示双侧对称部位CBF比值为0.92-1.09;检出所有患者脑动脉不同程度狭窄,TOF-MRA显示动脉重度狭窄病变12例(23.07%),中度狭窄病变25例(48.07%),轻度狭窄15例(28.84%);ASL显示相应区CBF重度减低12例(23.07%),中度减低18例(32.69%),轻度减低6例(11.53%),两种检查方法仅针对轻度病变检出率有统计学差异(P<0.05)。结论:脑动脉中度及重度狭窄可导致所支配区脑血流量相应下降;而脑动脉轻度狭窄与所支配区脑血流量下降不成对应关系,两种检查方法在诊断轻度脑缺血病变具有统计学差异。Objective: To study time of fly magnetic resonance artery imaging(TOF-MRA) and magnetic resonance arterial spin la- beling(ASL) perfusion imaging in patients with ischemic lesions in the brain artery stenosis and the application value of the relation- ship between cerebral bloo flow(CBF). Methods:From September 2013 to August 2014, 12 cases of normal volunteers and clinical di- agnosis of 52 patients of cerebral ischemia in craniocerebral routine scan based uplink TOF-MRA, ASL 3 dimensional scanning were collected to analyze the artery stenosis degree and the relationship between cerebral blood flow. Results; Normal volunteers-TOF MRA are normal, ASL show CBF ratio of 0. 92 to 1.09 in bilateral symmetry parts. All of the patients were checked out the different level of cerebral artery stenosis. TOF MRA showed severe stenosis artery lesions-12 cases(23.07%), moderately narrow lesions in 25 cases(48.07%), 15 cases(28. 84%), mild stenosis~ ASL showed the zone of corresponding CBF severely reduced in the 12 cases (23.07%), moderate decreased in 18 cases(32, 69%), slightly reduced in 6 cases(11. 53%). There is a statistically difference(P〈 0. 05) in mild lesion detection rate. Conclusions: Cerebral arteries in moderate and severe stenosis may lead to cerebral blood flow and dominated area decreased accordingly. And mild cerebral arteries narrow dominated area cerebral blood flow decreased with no corre- sponding relationship. Two test methods in the diagnosis of mild cerebral ischemia lesion degree showed significant differences statis- tically.
