摘  要:目的探讨慢性阻塞性肺疾病急性加重期(AECOPD)患者肺功能与血气分析、炎症指标的相关性。方法通过对我院2009年1月~2014年6月期间504例AECOPD患者回顾性调查,分析其血气分析、炎性指标以及肺功能的相关性。结果 Pearson相关性分析显示:AECOPD患者血气分析、炎症指标与肺功能参数显著相关,差异有统计学意义(P〈0.05),其中PH与GOLD分级呈负相关(r=-0.199,P=0.000),Sa O2与GOLD分级呈负相关(r=-0.143,P=0.003),Pa CO2与GOLD分级呈正相关(r=0.405,P=0.000),WBC与GOLD分级呈正相关(r=0.116 P=0.000),N与GOLD分级呈正相关(r=0.253,P=0.000),CRP与GOLD分级呈正相关(r=0.150,P=0.015),差异有统计学意义(P〈0.05)。结论血气分析(PH、Pa CO2及Sa O2)、炎症指标(WBC、N及CRP)水平与AECOPD患者病情严重程度密切相关,两者均可作为评估患者病情严重程度以及评估预后的参考。Objective To investigate the correlation of pulmonary function to blood gas and inflammatory biomarkers in patients with AECOPD. Methods The clinical data of 504 AECOPD patients from January 2009 to June2014,were retrospectively reviewed to analyze the correlation of pulmonary function to blood gas and inflammatory biomarkers. Results The Pearson correlation analysis showed that the correlation of pulmonary function to blood gas and inflammatory biomarkers was significant( P〈0.05). PH was negatively correlated with GOLD staging( r =-0. 199,P = 0. 000),Sa O2 was negatively correlated with GOLD staging( r =- 0. 143,P = 0. 003). Pa CO2 was positively related with GOLD staging( r = 0. 405,P = 0. 000),WBC was positively related with GOLD staging( r =0. 116,P = 0. 000),N was positively related with GOLD staging( r = 0. 253,P = 0. 000),and CRP was positively related with GOLD staging( r = 0. 150,P = 0. 015). Conclusion The level of blood gas analysis( PH,Pa CO2 and Sa O2) and inflammatory biomarkers( WBC,N and CRP) in patients with AECOPD can be used to evaluate the extent of illness and prognosis.
