基层医疗保健机构人工流产现状分析

摘　　要：目的了解基层医疗机构计划生育服务现状,为合理推广规范化流产后服务提供理论依据。方法对在湖南省长沙县妇幼保健院及贵州省安顺市妇幼保健院行人工流产的妇女进行计划生育相关问题问卷调查,并对结果进行统计分析。结果在长沙县妇幼保健院197例人工流产妇女及安顺市妇幼保健院172例人工流产妇女中,低学历者流产次数多于高学历者,差异有统计学意义(P<0.05);受教育水平、是否接受过避孕知识指导为人工流产的危险因素(P<0.05);2家医院人工流产妇女的主要妊娠原因均为未采用避孕方法。结论医务工作者在临床工作中应针对不同地域、不同生育阶段、不同流产经历、不同文化程度、不同经济水平的人群有针对性地普及避孕知识,指导其选择正确的避孕方式,降低避孕失败风险,提高避孕有效率,进而提高本地区妇女生殖健康水平。Objective To analyze the status quo of birth control in grass-roots health care institutions so as to provide a theoretical basis for the rational promotion and standardization of post-abortion services.MethodQuestionnaires about birth control were taken by women who had induced abortion in Changsha Hospital for Maternal Child Health Care of Hunan Province and Anshun Hospital for Maternal Child Health Care of Guizhou Province and the data were statistically analyzed.Results Among 197 cases from Changsha Hospital and 172 cases from Anshun Hospital,those with low levels of education had more abortion than those with high levels of education and the difference was statistically significant（P〈0.05）.The risk factors for induced abortion were levels of education and whether to have the guidance of contraceptive knowledge or not.No contraception was the main reason why pregnant women from the two hospitals had abortion.Conclusions Medical workers should disseminate targeted contraception knowledge to people from different regions,in different stages of pregnancy,with different abortion experiences and from different educational or economic levels and guide them to choose different contraceptives in order to reduce the risk of contraceptive failure,enhance the effectiveness of contraception and improve women＇s reproductive health in their region.
