不同波形电针对颈椎病颈痛的疗效观察

摘　　要：目的探讨电针治疗颈椎病疼痛疗效稳定持久的最佳波形。方法将2011年10月-2013年9月在重庆市北碚区中医院针灸推拿科就诊且符合纳入标准的90例颈型颈椎病患者随机分为单纯针刺组、疏密波电针组、连续波电针组,每组30例。选穴处方均取颈椎阳性反应平面双侧夹脊穴(取上、下2对)、双侧肩中俞、大椎、双侧中渚,单纯针刺组针刺后不加电针治疗,疏密波电针组及连续波电针组采用针刺后加不同波形的电针治疗。结果 3组患者治疗结束后,Northwick Park颈痛量表(NPQ)、疼痛视觉模拟评分(VAS)及健康状况调查问卷(SF-36)评分较治疗前均有明显改善(P<0.05);治疗结束后3个月时的NPQ,VAS及SF-36评分较治疗前也有明显改善(P<0.05);但疏密波电针组患者的NPQ,VAS及SF-36评分均优于同期其他2组患者。结论针灸对颈椎病颈痛有确切的临床疗效。疏密波电针治疗颈椎病颈痛的近期及远期疗效均优于单纯针刺及连续波电针治疗,验证了"疏密波是治疗疼痛性疾病的首选波形"。Objective To investigate optimal waveforms with stable and lasting efficacy of electrical acupuncture（EA）on neck pain of cervical spondylosis.Method 90 cases of patients with neck pain of cervical spondylosis received in Beibei District Hospital of Chinese Medicine from October 2011 to September 2013 were randomly divided into an acupuncture group,EA group of density wave,EA group of continuous wave,30 cases in each group.Selected acupuncture points were bilateral Jiaji points（the upper and lower pairs）of cervical positive reaction plane,bilateral Jianzhongshu,Dazhui and bilateral Zhongzhu.The acupuncture group was only given needling-acupuncture treatment with no electrical acupuncture and EA groups of density wave and continuous wave were given the treatment of different waveforms of electrical acupuncture after needling-acupuncture.Results Scores of Northwick Park Neck Pain Questionnaire（NPQ）,Pain Visual Analogue Scale（VAS）and 36-Item Short-Form Health Survey（SF-36）for patients in three groups were significantly improved after treatment（P〈0.05）;three months after treatment,scores of NPQ,VAS and SF-36 were noticeably improved（P〈0.05）,but scores of NPQ,VAS and SF-36 for patients in EA group of density wave were better than the other two groups.Conclusion Acupuncture has positive efficacy on neck pain of cervical spondylosis.Density wave of electrical acupuncture has better analgesic effect on the early stage and long term of neck pain of cervical spondylosis and can better improve patients＇ health status than acupuncture and continuous wave of electrical acupuncture,which proves that＂density wave is the optimal waveform to treat painful diseases.＂
