男性性功能症状评分诊断迟发性性腺功能减退症的临床研究

摘　　要：目的：探讨男性性功能症状评分诊断迟发性性腺功能减退症的有效性。方法：本研究选择的对象为2011年5月至2013年3月我市接受调查的1245例中老年男性,通过问卷调查的形式对1245例男性的一般资料、男性雄激素缺乏问卷表（ADAM）以及老年男性症状表（AMS）进行调查,同时测定调查者黄体生成素（LH）、游离睾酮（f T）、睾酮（TT）以及性激素结合球蛋白（SHBG）的水平。结果：1245例男性调查问卷共获得1196例有效调查问卷,调查问卷有效率96.1%,从年龄分布来看调查者的年龄多分布在50~69岁的年龄段;各个年龄段的ADAM筛查LOH阳性率与AMS筛查LOH阳性率相比有显著性的差异,两组进行比较均有显著性差异（P〈0.05）,ADAM筛查LOH平均阳性率与AMS筛查LOH平均阳性率相比有显著性的差异,两组进行比较有显著性差异（P〈0.05）;SHBG、TT在ADAM与AMS筛查阳性与阴性组之间的水平无显著性差异（P〉0.05）;f T、LH在ADAM与AMS筛查阳性与阴性组之间的水平有显著性差异（P〈0.05）。结论：综上所述,采用AMS与ADAM症状评分结合f T以及LH水平能够准确的对迟发性性腺功能减退症进行诊断,值得对其进一步研究。Objectives： To investigate the validity of male sexual function symptom score on the diagnosis of tardive hypogonadism. Method： 1,245 medium- elderly men,who were surveyed in our city from May 2011 to March 2013,were selected. Through a questionnaire survey,general information,male androgen deficiency questionnaire（ ADAM） and older men symptoms table（ AMS） of them were investigated and luteinizing hormone（ LH）,free testosterone（ f T）,testosterone（ TT） and sex hormone binding globulin（ SHBG） levels of them were detected at the same time. Results： Among the 1245 cases surveyed,1196 cases of valid questionnaires were obtained,a survey efficiency of 96. 1%. In terms of age distribution,the majority was 50- 69 years old; the difference in ADAM screening LOH positive rate and AMS screening LOH positive rate among each age group was significant（ P〈 0. 05）. The difference in ADAM screening LOH average positive rate and AMS screening LOH average positive rate was statistically significant（ P〈 0. 05）; SHBG and TT levels of ADAM and AMS positive and negative screening groups were of no significant difference（ P〉 0. 05）; f T and LH levels of AMS positive and negative screening groups was of statistically significant difference（ P〈 0. 05）. Conclusion： In summary,the use of AMS and ADAM symptom scores combined with f T and LH levels can accurately diagnose the delayed hypogonadism,which is worth further study.
