天津市2008-2013年5种STD的流行病学分析

摘　　要：目的了解天津市性传播疾病（STD）的流行趋势及流行特点,为制定控制对策提供科学依据。方法对2008-2013年天津市5种STD疫情报病资料进行发病率等流行病学指标的统计分析。结果 2008-2013年,全市5种STD发病呈下降趋势,由2008年的7010例（报告发病率为62.87/10万）降至2012年的4895例（报告发病率为36.14/10万）,年均下降12.93%;2013年疫情略有回升,报告发病率较2012年增长了2.94%。天津市STD的流行病谱发生了改变：梅毒始终居首位;2013年尖锐湿疣和淋病升至二、三位,生殖道沙眼衣原体感染降至第四位,生殖器疱疹列末位。全市STD发病较高的地区为市内六区和滨海新区,其各年病例报告合计数占全市STD报告总数的比例均超过60%以上。男女STD病例的比例缩小,由2008年的1.44∶1缩小至2013年的1.17∶1。20~49岁的性活跃人群发病率最高,约占全部STD病例数的60%以上。结论天津市STD流行趋势总体呈下降趋势,但2013年出现小幅回升;STD报告病例地区差异大;60~岁组发病率逐渐增加。应加强STD监测,有针对性地进行STD防治工作。Objective To understand the trends and epidemiological features of sexually transmitted disease（STD）in Tianjin during 2008-2013,and provide scientific basis for making control strategies.Methods The casereport data of notifiable and monitoring STDs during the period of 2008-2013 were analyzed with epidemiological methods.Results ①During 2008-2012,the morbidity of STDs in Tianjin showed an decreasing trend.The number of reported STDs cases steadily decreased from 7010（incidence 62.87 per 100 000）in 2008 to 4895（incidence 36.14 per 100 000）in 2012.The average annual decrease rate was 12.93%.However in 2013,the epidemic rose slightly and the reported incidence increased 2.94% compared with 2012.②The high-incidence area was the city＇s six districts and Binhai new area district whose average annual reported cases of STDs accounted for over 60% of total cases.③The male to female ratio of STDs decreased from 1.44∶1in 2008 to 1.17∶1in 2013.STD incidence rates were the highest among sexually active individuals aged from 20 to 49years,and the reported STD cases in this age group accounted for over 60% of total cases.Conclusion The incidence of STDs is in a decrease trend in Tianjin,except 2013.The case-report data from different areas showed significantly differences.The cases of STDs in the elderly group increases gradually.Actions should be taken to enhance the monitoring and the prevention of STDs.
