摘　　要：目的:防范调剂差错,杜绝药疗事故发生。方法:分析门诊药房容易发生调剂差错的类型和原因,采取相应对策,建立规范的调剂操作规程和行之有效的监督管理机制。结果与结论:通过分析、总结差错类型和原因,并制订相应对策进行干预,能最大限度地减少调剂差错,确保患者用药安全。药学人员在工作中应高度重视调剂差错问题,规范调剂规程、有效的管理制度和对人员素质的培养,以避免调剂差错的发生。OBJECTIVE： To prevent dispensing errors and eliminate medication accident. METHODS： The types and the cause of outpatient pharmacy dispensing errors were analyzed and corresponding measures were taken,and standard operating procedures and effective supervision and management mechanism were established. RESULTS CONCLUSIONS： By analyzing and summarizing the type and cause of dispensing errors and developing appropriate countermeasures can help reduce the dispensing errors to a maximal degree and ensure patients＇ medication safety.Pharmacy staff should attach great importance to dispensing error, standard dispensing procedures, effective management system and the quality training of personnel in order to avoid the occurrence of dispensing error.
