摘  要:目的了解金黄色葡萄球菌临床分离株对红霉素和克林霉素的耐药性,检测红霉素对克林霉素诱导耐药的发生率。方法根据美国临床实验室标准化协会(CLSI)相关解释标准,用头孢西丁筛查试验检测耐甲氧西林金黄色葡萄球菌(MRSA),同时,利用克林霉素诱导试验(D试验)分析红霉素对克林霉素诱导耐药表型。结果对红霉素和克林霉素同时耐药的MRSA和对甲氧西林敏感的金黄色葡萄球菌(MSSA)分别为245株和22株,分别占各自菌株总数的65.7%和29.7%;对红霉素耐药而克林霉素敏感的MRSA和MSSA分别为112株和20株,其中,D试验阳性的MRSA和MSSA分别占所在组别各自菌株总数的84.8%和55.0%。结论临床微生物实验室开展D试验,对于检测金黄色葡萄球菌中红霉素对克林霉素的诱导耐药性,指导临床医生合理使用抗生素具有重要意义。Objective To investigate the resistance of clinical Staphylococcus aureus isolates to erythromycin and clindamycin and detect the incidence of clindamycin- induced resistance by erythromycin. Methods According to the standards of Clinical and Laboratory Standards Institute( CLSI),meticillin- resistant Staphylococcus aureus( MRSA) were detected by cefoxitin screen test. Meanwhile,the inducible resistance phenotype of erythromycin to clindamycin was analyzed by clindamycin induced test( D- test). Results MRSA and MSSA co- resistant to erythromycin and clindamycin were 245 strains and 22 strains,account for 65. 7% and 29. 7% respectively. In addition,erythromycin- resistant and clindamycin- susceptible strains of MRSA and MSSA were 112 and 20 respectively. Additionally,MRSA and MSSA with D- test positive account for 84. 8% and 55. 0%respectively. Conclusion D- test is used for detection of the inducible resistance of erythromycin to clindamycin in Staphylococcus aureus,which will contribute to guide physicians to select antibiotic rationally.
