经尺骨鹰嘴截骨手术入路治疗C_3型肱骨髁间骨折  

EFFECT OF OLECRANON OSTEOTOMY APPROACH FOR TREATMENT OF TYPE C3 INTERCONDYLAR FRACTURES OF THE HUMERUS

摘　　要：目的评价双钢板固定手术治疗肱骨髁间骨折的临床疗效。方法该组患者31例,男19例,女12例,年龄20-64岁,平均38.2岁;高处坠落伤10例,交通事故伤16例,重物压砸伤5例。均为C3型肱骨髁间骨折,全部经尺骨鹰嘴入路应用双钢板内固定治疗,用Jupiter肘关节功能评分标准行术后患肘功能评分。结果 31例均获得随访,术后平均随访时间为15个月。平均愈合时间为4个月。所有截骨块全部愈合,平均愈合时间8周。按改良Jupiter肘关节评分标准：优24例,良4例,可2例,优良率90.3%。结论经尺骨鹰嘴截骨入路双钢板固定治疗C3型肱骨髁间骨折具有固定牢靠、并发症少、满意率高等优点,可早期进行功能锻炼,是一种较好的治疗肱骨髁间骨折的内置物。Objective To explore the clinical efficacy for treatment of humeral intercondylar fracture with the two steel plate.Methods Thirty-one patients which were consisted of twenty-one men and ten women,age 20-64,were divided into three groups：10 cases of fall injury,16 cases of traffic accidents,6cases of heavy crash injury;All patients were humeral intercondylar fracture of type C3,adopted chevron osteotomy of the olecranon approach and internal fixation with the two steel plate,and postoperative elbow join function score was performed with Jupiter elbow joint function score standard.Results Totally 31 patients were all followed-up,the average follow-up period was 15 months.The average healing time was4 months.All bone cutting piece were healed,with an average healing time of 8 weeks.According to the Jupiter elbow grading standard,the results were excellent in12 cases,good in4 cases and ok in2 cases,the perfect rate was 90.3%.Conclusion Chevron osteotomy of the olecranon with the two steel plate is a better way to treat distal humeral fracture of type C3,the advantages is strong fixation,less complication and high satisfaction rate,early functional exercise.
