新农合及妇幼卫生项目补偿对妇幼卫生服务的影响研究

摘　　要：目的研究新农合妇幼卫生服务政策对妇幼卫生工作的影响。方法选取云南省隆阳区、施甸县、腾冲县、龙陵县和昌宁县,分别收集2011-2013年5县区的新农合实施方案,调查5县区新农合妇幼卫生服务政策,包括报销农村孕产妇住院分娩所产生的、额定的医疗服务项目费用,同时对发生危急抢救孕产妇所产生的费用,按发生费用的比例给予报销情况。对相关人员进行访谈,对当地孕产妇及经产妇进行问卷调查。结果 5县区住院分娩率逐年提高,2013年平均达到99.6%,高危孕产妇住院分娩率达到100%;孕产妇死亡率和新生儿死亡率较2011年相比,分别下降12.0和2.3个百分点;同时也降低了辖区无指征剖宫产率;各县的孕产妇系统管理率和高危孕产妇管理率均有上升。结论通过政策实施提高了当地孕产妇住院分娩率,基本消除了新生儿破伤风的发生,降低孕产妇和新生儿死亡率,提高县、乡级产科服务能力。对控制剖宫产率起到一定的促进作用。同时,孕产妇健康管理和0~6岁儿童健康管理项目支出由基本公共卫生补助经费进行补偿。两种经费的合理分配、使用,对妇幼卫生工作起到事半功倍的作用。Objective To new farming and policy study of the influence of maternal and child health services. Methods To select the long Yang district, ShiDian county of yunnan province, tengchong county, longling county and chang zhengning county, were collected from 2011 to 2013, five counties district agriculture and implementation plan, interviews with relevant personnel, the local maternal and multipara questionnaire.
