摘　　要：目的了解中老年高血压患者血压晨峰(morning blood pressure surge,MBPS)的发生情况,为制定针对性的护理对策提供依据。方法便利抽样法选择2012年6月至2013年12月在广州军区广州总医院进行体检的646例中老年高血压患者为研究对象,按年龄将其分为中年组、老年组(包括低龄老年、高龄老年),对所有患者进行24h动态血压监测(ambulatory blood pressure monitoring,ABPM),了解MBPS的发生率及幅度。结果 646例患者中,共有117例(18.1%)发生MBPS,幅度为(22.39±13.59)mmHg;老年组患者MBPS的发生率和幅度均高于中年组(χ2/t=8.761/2.348,均P<0.05),低龄老年与高龄老年患者MBPS的发生率和幅度的差异均无统计学意义(均P>0.05);MBPS组患者的诊室收缩压(systolic blood pressure,SBP)和舒张压(diastolic blood pressure,DBP)与非MBPS组比较,差异均无统计学意义(均P>0.05)。结论年龄对中老年高血压患者的MBPS发生存在影响,医护人员要加强对中老年高血压患者晨峰现象的重视,给予恰当的护理干预措施。Objective To investigate the incidence of morning blood pressure surge （MBPS） in middleaged and elderly hypertensive patients,and propose nursing strategies of prevention.Methods By conven- ience sampling, 646 cases middle-aged and elderly hypertensive patients were selected and divided into middle-aged group and elderly group （less old age groups and very old group）, and with 24-hour ambulatory blood pressure monitoring to understand the incidence rate and magnitude of MBPS.Results Among 646 cases of patients,ll7 cases（18.1%） had MBPS with （22.39±13.59）mmHg magnitude.The incidence rate of MBPS and magnitude in elderly group were both higher than which in middle-aged group（x^2/t=8.761/ 2.348,all P（0.05）,while no statistical significance appeared ＇between the less old age groups and very old group（P 〈0.05）.There was also no statistical significance between MBPS group patient and non-MBPS group of systolic blood pressure and diastolic blood pressure （all P〉0.05）.Conclusion The age influences the incidence of morning blood pressure surge in middle-aged and elderly hypertensive patients, and medical staff should pay attention to it and provide proper nursing interventions.
