锁定接骨板治疗老年骨质疏松性跟骨骨折回顾性临床研究

摘　　要：目的总结锁定接骨板治疗老年骨质疏松性跟骨骨折的临床效果。方法使用锁定接骨板治疗老年骨质疏松性单足跟骨骨折82例,术前行跟骨侧、轴位X线片和CT检查,并采用双能X线测定骨密度,明确骨质疏松诊断。手术前后对Bohler角、Gissane角、跟骨水平全长、丘部总高、轴长和体宽等多项指标进行比较。结果本组随访9~15个月（平均11个月）。骨折全部愈合,平均愈合时间3.5个月。术后切口感染1例,皮缘坏死3例,经治疗后均愈合,2例术后8个月出现明显的距下关节炎。手术时间平均55 min。根据Maryland足部评分标准：优48例,良26例,可6例,差2例,总优良率90.2%。结论采用跟骨锁定接骨板治疗老年骨质疏松性跟骨骨折,固定牢靠,并发症少,值得临床推广应用。Objective To analyze the results of treatment with the locking plate in senile patients with osteoporotic calcaneal fractures. Methods Eight-two cases of simple calcaneal fracture with osteoporosis were treated with locking plate. All patients were examined with X- ray, CT and dual- energy X- ray absorptiometry measurement before surgery. Bohler Angle, Gissane Angle, span, high total calcaneal level were measured and the function of foot was evaluated by Maryland foot scoring system before and after surgery. Results Eight-two cases were followed-up. Bone union occurred in all cases. The average time of fracture healing was 3.5 months. One case had incision infection, 3 cases had skin necrosis. The necrosis healed after treatment. Eight months later, 2 cases has evident subtalar arthritis. The average operation duration was 55 mins. According to the Maryland foot scoring criteria, the excellent and good rate was 90.2%. Conclusion Locking plate is an ideal method to treat osteoporotic calcaneal fractures in senile patients. It is worth to apply in clinic because of its stable fixation and less complications.
