针刺华佗夹脊穴和督脉穴对强直性脊柱炎患者脊柱功能活动度和疼痛的影响

摘　　要：目的探讨针刺华佗夹脊穴和督脉穴位对强直性脊柱炎患者脊柱功能活动度和疼痛的影响。方法将2010年8月至2014年8月河北省涉县中医院收治的118例强直性脊柱炎患者按随机数字表法分为观察组(60例)和对照组(58例)。观察组患者采用针刺华佗夹脊穴和督脉穴位治疗,对照组采用柳氮磺胺吡啶片治疗。10 d为1个疗程,共治疗3个疗程。比较两组患者脊柱功能活动度和疼痛的改善程度。结果治疗后两组患者功能活动和病情活动明显优于治疗前,差异有统计学意义(P<0.05);治疗后,观察组检测项目穿袜、坐位直立、触及高位、平卧起身和体能活动优于对照组(P<0.01),身体疲倦程度、关节疼痛程度和关节僵硬程度优于对照组(P<0.05或P<0.01)。观察组患者总有效率为91.6%(55/60),对照组患者为79.3%(46/58),观察组临床疗效优于对照组(P<0.05)。结论针刺华佗夹脊穴和督脉穴对强制性脊柱炎患者功能活动度有很好的改善,且能降低患者疼痛程度和疾病的活动性,值得在临床上广泛应用。Objective To discuss the effects of acupuncture on Jiaji point and Dumai point on the spinal function activity and pain of patients with ankylosing spondylitis.Methods A total of 118 patients with ankylosing spondylitis admitted in She County Hospital from Aug.2010 to Aug.2014 were divided into control group（58 cases） and observation group（60 cases）.Patients in the observation group were treated with acupuncture on Jiaji points and Dumai points,while patients in the control group were treated with sulfasalazine tablets.The improvement of spinal function activity and pain of the patients in both groups was compared.Results The function activity and disease activity of both groups were obviously improved after treatment than that before treatment,there were statistically significant differences（P〈0.05）.Those activities such as wearing socks,sitting erect,touching highs,and recumbent up of patients in the observation group were greater than those in the control group after treatment（P〈0.05）.And the degrees of body fatigue,joint pain and anchylosis of patients in the observation group were better than those in the control group（P〈0.05）.The total effective rate of the observation group was 91.6%（55/60）,while 79.3%（46/58） in the control group.Conclusion The acupuncture treatment on Jiaji point and Dumai point has great improvement on the function activity of patients with ankylosing spondylitis,and decrease the pain degree and diseases activity,therefore,it is worthy of clinical application.
