国产3%盐酸氯普鲁卡因联合1%罗哌卡因用于下肢手术硬膜外麻醉的效果观察

摘　　要：目的探讨国产3%盐酸氯普鲁卡因联合1%罗哌卡因用于下肢手术硬膜外麻醉的效果。方法选择2010年1月至2013年6月在廊坊市第四人民医院行下肢手术的患者90例,根据随机数字表法将患者分成两组,每组45例。观察组注入3%盐酸氯普鲁卡因和1%罗哌卡因各10 m L,对照组分3次注入3%盐酸氯普鲁卡因共20 m L。观察两组麻醉起效时间、痛觉减退时间、痛觉消失时间、运动阻滞时间、运动恢复时间、痛觉恢复时间;记录麻醉后10、20、30 min时平均动脉压(MAP)、心率以及脉搏血氧饱和度。结果两组麻醉起效时间、痛觉减退时间、痛觉消失时间和运动阻滞时间比较差异无统计学意义(P>0.05),观察组运动恢复时间和痛觉恢复时间长于对照组,差异有统计学意义(P<0.05);麻醉20 min后观察组MAP较对照组显著降低,差异有统计学意义(P<0.05),其他指标比较差异无统计学意义(P>0.05)。结论国产3%盐酸氯普鲁卡因联合1%罗哌卡因用于下肢手术硬膜外麻醉起效快,作用时间长,对血压的影响小。Objective To investigate the effect of epidural anesthesia using domestic 3% chloroprocaine combined with 1% ropivacaine for lower limb surgery.Methods A total of 90 patients underwent lower linb surgery in Langfang Fourth People＇ s Hospital from Jan.2010 to Jul.2013 were randomly divided into the observation group（n = 45） and control group（n = 45）.The mixture of 3% chloroprocaine and 1% ropivacaine was injected into patients of the observation group and 3% chloroprocaine was injected into patients of the control group.The onset time of hypoalgesia,pain decreased time,pain disappearance time,motion block and recovery time,as well as pain recovery time of the two groups was observed.Mean arterial pressure（MAP）,heart rate and pulse oxygen saturation at 10,20,30 min after anesthesia were recorded.Results The onset time of hypoalgesia,pain decreased time,pain disappeared time and motion block time of the two groups had no statistically significant difference（P〉0.05）,the motion recovery time and pain recovery time of the observation group was longer than those of the control group,the difference was statistically significant（P〈0.05）.At 30 min after anesthesia,BAP of the observation group decreased more significantly than the control group,the difference was statistically significant（P〈0.05）,while other indicators had no statistically significant differences（P〉0.05）.Conclusion Epidural anesthesia using domestic 3% chloroprocaine combined with 1% ropivacaine for lower limb surgery can work fast,prolong the duration time,and has little effect on blood pressure,with no obvious side effects.
